Michael Cropp, MD, Board Chair of the P2 Collaborative of Western New York convened the meeting at approximately 10:10am.

P2 Updates

P2 2009 Conference – is scheduled for Thursday, September 24 and Friday, September 25, 2009. The conference this year will feature three tracks: Equity; Healthy Lifestyles; and, Regional Quality Improvement. The tracks will begin by taking a look at the national perspective and work their way down to the local level. If you are interested in becoming a sponsor, please contact Tina Chaudhry at .

D’Youville Mini Conference – The first discussion around public reporting occurred at D’Youville College in late March. Nearly sixty people were in attendance to hear Michael Parkinson, MD, and Shoshanna Sofaer, discuss the benefits of public reports.

o  PSU Survey – A survey was sent out by Penn State University in order to help P2 gauge how we are doing as a collaborative. A similar survey went out about two years ago. Please take a moment to complete.

Collaborative Updates

Performance Measurement

Jim Notaro, WNY Quality Measurement Collaborative (WNYQMC), presented updates from the WNYQMC. A NYSDOH grant has facilitated the three regional health plans to collaborate and aggregate medical and pharmacy claims data. The purpose of aggregating data is to create a quality measures report for the purpose of quality improvement. These reports will be made available to primary care physicians through a web portal. The physician will be able to view his/her aggregated scores alongside the national average and then be able to drill down to individual patient reports. 2008 data will be available to physicians in August 2009. By fall 2009 data will be available for the first quarter 2009.

Regional Quality Improvement

P2, with the support of the Community Health Foundation of Western & Central New York (CHFWCNY), is tackling regional quality on two fronts: (1) By developing and building a uniform quality improvement skill set within WNY; (2) By developing an understanding of Western New York’s unique populations and who is being over-looked in health care.

Ann Monroe, President of the CHFWCNY, discussed opportunities that have been available in efforts to build QI capacity.

·  In February 2009, two 2-day workshops, Fundamentals of Quality Improvement, were held. Over two hundred individuals, from hospitals, clinics, long term care facilities, etc, participated in these workshops.

·  The Fundamentals of Quality Improvement workshop leaders are offering follow-up webinars. The first webinar is scheduled to occur at the end of May. During this webinar, three organizations (Medina Memorial Hospital, Roswell Park Cancer Institute, and Elderwood) will present on the progress of the quality improvement initiatives they chose to embark upon during the session. The second webinar is set to occur in July and will address how to engage and spread quality improvement.

·  Improving Care and Caring was a series of six quality improvement workshops that targeted facilities that offered long-term care. This series had over 75 participants. A similar series of quality improvement workshops will be developed for hospitals and ambulatory care.

In order to address inequities in healthcare, the WNY Health Equity Coalition was formed. The members of the work group were surveyed by Community Catalyst regarding their perceptions of health equity in Western New York. On May 4th, the survey results were presented to the coalition and a visioning session took place, from which 4 subcommittees were formed. For more information, contact Dawn Tierney at .

Consumer Engagement

Tracy Sawicki, Consumer Engagement Team Leader for the P2 Collaborative of WNY, reported on consumer engagement updates.

·  RWJF AF4Q Consumer Engagement Focus – The Robert Wood Johnson Foundation recently refocused their consumer engagement goals. Consumer engagement has evolved to: Consumers will access and use health and comparative performance information to make health care decisions at key points.

·  Patient Empowerment Training (PET) – The Consumer Engagement Associates (CEAs) in partnership with the National Partnership for Women & Families (NPWF) has brought PET sessions to over 65 individuals in WNY. This training session, provided by NPWF, teaches patients not only what quality healthcare is, but to expect it and to ask for it. ·  Diabetes Resource Guide – is currently available online at www.thinkbright.org/myhealthcounts and www.p2wny.org.

·  Stanford Chronic Care Model (CDSMP) – This program assists patients with chronic diseases to take charge and self manage their disease. It’s an evidence-based six week program (one session per week). The P2 Collaborative is currently looking to unite efforts region-wide.

My Health Counts – Show 3

Dawn Hamilton, Pam Johnson, Christy May, and Dave Rotterman, WNED-TV, gave updates on My Health Counts! My Health Counts! Show 3 focusing on quality and features Susan Hunt, Ann Monroe, and Michael Parkinson. Show 3 is in the final stages of editing and will premier on Wednesday, July 1, 2009, at 9:30pm. Development of the second round of shows will begin in the near future. Chronic heart disease, diabetes, chronic lung disease, and hypertension will be the focus of the next shows. In addition to the television segments, WNED ThinkBright TV has been developing an outreach plan utilizing the web. A website with complimentary materials and tools has been created.

Transforming Care at the Bedside (TCAB)

Medina Memorial Hospital Health Care System and the Erie County Medical Center, were recently selected by the RWJF to participate in TCAB, a new effort to improve the quality of care in hospitals. This is a 3 year initiative with the goals to improve the care patients receive, increase the retention of quality nurses, improve the patient experience, and increase effectiveness of the staff. Representatives from both hospitals were available to report on the status of this exciting initiative.

·  Medina Memorial Hospital – Brian Banas reported on the efforts of Medina Memorial. Hospital representatives attended a RWJ sponsored workshop on March 3 – March 6, 2009. During this conference, the representatives were educated in a number of quality improvement tools, including rapid cycle testing. At Medina, the initiative is currently being piloted in the Medical-Surgical unit. The successes will be spread throughout the hospital. To improve communication between nurses/physicians, use of walkie talkies and white boards are currently being tested.

·  Erie County Medical Center (ECMC) – Bonnie Glica reported on the efforts being made at ECMC. The staff at ECMC chose to focus on pressure ulcer reduction. In order to address this issue, ECMC recently purchased new beds. In addition to specialized beds, nurses are making hourly rounds during which they turn the patients and have patients monitor pain.

WNY Community Health Planning Institute (WCHPI)

P2 was recently awarded the NYSDOH HEAL 9 grant. Ann Monroe, CHFWCNY, presented on the status of this grant. P2 is currently forming the Steering Committee of the WCHPI. The WCHPI will be conducting community-driven, population-based planning. Several positions – a director, data analyst, and planning analyst – are available. New York State Health Foundation (NYSHF) $35M Diabetes Campaign

Deb Zahn, NYSHF, presented their $35M Diabetes Campaign and how they could collaborative with P2. The Diabetes Campaign is a 5 year initiative that focuses on three main areas:

·  Improving clinical care and outcomes by focusing on primary care by physicians, clinics and hospitals

·  Mobilizing communities by supporting diabetes prevention, screening and management programs that already exist

·  Promoting policy by pursing reimbursement policies and community strategies

To engage the region, the NYSHF is taking the following steps:

·  A number of conversations are being held to discover what the providers would like and what the NYSHF can do to help. They have contracted with Bridges 4 Excellence to conduct a survey to discover what obstacles providers are facing while caring for patients with diabetes, as well as discover incentive programs currently in place for providers. Representatives from the health plans have been contacted and surveyed. On May 7th, the NYSHF and NYDC will hold a conversation with providers. The NYSHF is interested in the work currently being done by the PEAs and CEAs, specifically what they are finding within the community.

·  Social marketing campaigns are being embarked upon – one amongst physicians and one with faith based organizations.

·  Data driven population-based community planning is occurring.

Center for Health Care Strategies (CHCS)

Nikki Highsmith, CHCS, presented the work currently being done by CHCS and the support they could offer P2. CHCS focuses their efforts on low income, Medicaid, vulnerable populations. Medicaid data could augment work already being done in the region. Through their connections with the NYSDOH, CHCS could provide P2 with Medicaid fee-for-service and race/ethnicity/language data; technical assistance for measurement and reporting of quality measures; and, seed grants. CHCS would like to assist P2 in quality improvement initiatives across the region.
