TUSC REAL PREMIER – COMMITMENT FORM – 2016 to 2017

TUSC believes that at this level it not only takes skill and athleticism but commitment in order for players to get the most out of their REAL experience. The commitment expectations are outlined as follows - A player’s failure to keep their commitments may lead to: reduced playing time, NOT being rostered for a game, and/or being movedto practice player status for an indefinite period of time. All player placements and team league placements are based on the combined evaluations and decisions of the Boys D.O.C, Girls D.O.C, Premier Commissioner and the TUSC president.

I accept and understand that the player fees are payable in full on the due dates provided and that there will be no refunds, or partial refunds, under any circumstances.

I am aware that player uniform expenses are ‘in addition to’ and not part of player fees.

During the winter season I am committed to attending one training session per week andI commit to play in at least one of up to two tournaments during the winter.

I am aware that any sessions of additional Winter League play or extra tournament participation might be an addedcost. If it is, payment will only be split between players who wish to participate in the activity.

During the spring season I am committed to attending two practices each week and games on the weekends.

I will be available to play in up to two tournaments in the spring. One tournament will be onMemorial Day weekend.

Payment Deadlines –Full Player fee is $1200 for Winter Spring seasons

Practice Player fee is $750 for Winter & Spring seasons

June Try-outs - The club reserves the right to withdraw my offer if I have not paid my deposit by the June Try-out offer deadline. June deposit is $300. Check # _____

32 Ridgeview Ave, Trumbull, CT 06611. Or payments can be made via PayPal.

I understand if I miss any of these payment deadlines, an additional $25 per month will be added to my outstanding payment balance.

If, for any reason, the club cannot field a team for me to play on, any payments that I have made will be refunded to me by the club, less costs incurred by club for services rendered up to that point. A minimum of $99 will be kept by the club to cover the cost of insurance, cluband administration fees.
