Review of Transmission Group SIRS Reports from Previous Week:

  • SIRS 1231-2010 (NEEWS Project): Truck/Lowboy trailer transporting construction equipment on public roadway between construction site and storage yard clipped and knocked down a traffic control pole to the ground (no injuries). Pole was located on the corner of Suffield/Springfield street (PAR).

Review of Significant NU SIRS Reports from Previous Week:

  • SIRS 1235-2010 (Yankee Gas):employee was fined for having an over loaded dump truck
  • SIRS 1234-2010 (WMECO Elec. Maint.): Substation crew was performing switching for maintenance of vacuum circuit breakers at Pleasant Substation, 16B. There were two breakers to be maintained with separate switching orders for each breaker. In the process of switching the 16B1 line to the alternate bus, the 16B5 breaker was opened in error. The crew inadvertently utilized the wrong switching order. Because of the alternate bus configuration, there was no customer outage.
  • SIRS 1224-2010 (CL&P Lines): a 2 man crew was assigned to set a new pole. When the crew arrived on the scene, they had a tailboard and as part of the tailboard the crew discussed the CBYDticket. The crew visually verified that the area was marked and where the pole was to be set was clear of any hazards. The crew set up the digging truck and proceeded to dig the hole. The auger reached approximately 2 feet into the ground when the auger struck a 2" gas pipe. The crew immediately secured the vehicle, notified dispatch, and proceeded to evacuate the surrounding buildings due to the blowing gas. They then notified local supervision of the incident. The area was marked NOYG indicating that there was no gas in the area.

Job Hazard Assessments

Transmission Safety is currently rolling out information to all Transmission Maintenance and Work Management field employees on Job Hazard Assessments (JHA). Procedure M8-MT-2005 (JHA Process) was developed that explains the process for conducting a JHA. The procedure is being reviewed as part of the roll-out processthat will be completed on 9/30/10.

  • A JHA is a technique that focuses on the job tasks as a way to identify hazards before they occur. They are developed and documented as part of the pre-planning process and are communicated to workers prior to the start of work during daily safety briefings.
  • JHA defines the relationship between:
  • the worker
  • the task
  • the tools, and
  • the work environment
  • After identifying the hazards, crews take the appropriate steps to eliminate or control hazards with barriers to an acceptable risk level.
