This research is generously supported by the Diocese of Worcester, Mary Stevens Hospice, the British Association of Psychological Type, the Foundation of St Matthias and the Mylne Trust.

To: Colleagues in the Diocese of Birmingham

This booklet is intended to help you reflect on the connection between personality and end of life care. This is one of a number of research projects aiming to improve health and social care at the end of life.Your contribution is important and appreciated very much. You are not asked to write your name on the booklet so replies are completely confidential.

Please note that the content of the questionnaire concerns a very sensitive area of life and experience. Please think carefully before attempting it if you feel that you might be unduly vulnerable on this topic at the moment.

With my thanks, David Knight, Chaplain, Mary Stevens Hospice, Diocese of Worcester.
