SPPA 5530 Assignment: Case Report

General Instructions: Assimilate data on a hypothetical stuttering case and construct a complete assessment report with full therapy recommendations. A sample assessment report can be found in your textbook. You may use that as a model. Be sure to include a specific set of treatment recommendations for management of the case.

Due Date: Wednesday, December 14, 2016 (at final exam). Early assignments are welcomed. Late assignments will not be accepted.

Credit Value: 10 percentage points allotted for course assignments. You MUST complete the assignment on your own. Any evidence of collaboration will result in immediate referral to the Office of Student Conduct.

Materials:

  • Audio recording of standard reading passage “sppa 5530 sample.mp3” – This file is located in the “Assignments” page of the course website next to the link for this document. There are two possible ways to access the recording.
  • Clicking on the audio file link MAY automatically load an audio player, allowing you to listen to the sample directly from the website. However, this will depend on your computer and web browser settings.
  • Using a windows computer, right click the mouse on the file, and you will be given an option to save the file to your computer. Once downloaded, you can listen to it on the computer using any type of media software (e.g. Windows Media Player) or transfer to a personal listening device.
  • A transcript of the reading passage with specific instructions for what to do with the speech sample are on the last pages of this document.
  • NOTE: I do recognize that the audio recording is of an adult female even though the case study is for an adult male. This is the result of me mixing and matching different assignment materials from year to year. It will have no bearing on how you tackle the assignment.

  • Completed OASES form (on course website) AND OASES manual (on reserve at LRC)
  • Using the completed OASES form and the OASES manual for reference, score the OASES for this client. Include the key information from the OASES in the written assessment report. Include the scored OASES form with your assessment report.

Brief Summary of Background Information

  • 24 year old male
  • Unmarried
  • Occupation: drives a delivery van where he drops off and picks up farm equipment parts to a small number of farm equipment dealerships in the region.
  • Work involves very little interaction with new people and only brief encounters with the various inventory clerks at the different dealerships
  • Feels like his stuttering limits him in his ability to move up in the company. Would like to work at the parts counter interacting more with customers.
  • Hobbies include bowling, fishing and hunting
  • Avoids engaging in social activities that involves talking
  • Reports to be embarrassed and ashamed about his speech
  • Would like to have a relationship but believes his speech prevents him from meeting anyone.
  • Wants to become fluent so he can meet someone to begin a relationship.
  • Enjoys talking to the small group of friends, but is unwilling to engage in unfamiliar social situations.
  • Claims he began stuttering around age 5 years.
  • Received speech therapy from 3rd to 8th grade – remembers working on ‘slowing his speech down’
  • Uncle and grandfather both have a history of stuttering
  • He has two older sisters (neither stutter).
  • No major health problems. Only significant medical history is a broken leg at age 15. Never been hospitalized and does not currently take any medication.

Task:As part of the assessment report, you need to perform a speech sample analysis of a short reading passage. While this sample is limited to oral reading, and too short in length to be a valid representation of stuttering, it will allow you to gain some experience with the process.

  1. Below is an orthographic transcription of the reading passage used in the sample. While listening to the audio sample, use the orthographic transcription to mark locations and types of disfluencies and other noteworthy observations. While you are free to use any marking method you would like, make sure to include a legend so that I can understand your process. One option is to use the acronyms in the table below.
  2. Complete the table below with the appropriate information and make the necessary calculations. This table should be included in the assessment
  3. Attach to the assessment report the marked up transcript below and any pages showing the work you did to make the calculations for the measures.

When the sunlight strikes raindrops in the air, they act like a prism and form a rainbow. The rainbow is a division of white light into many beautiful colors. These take the shape of a long round arch, with its path high above, and its two ends apparently beyond the horizon. There is, according to legend, a boiling pot of gold at one end. People look, but no one ever finds it. When a man looks for something beyond his reach, his friends say he is looking for the pot of gold at the end of the rainbow.
