1. Letters of Recommendation

It is important that references be carefully selected. Choose individuals who are able to make the most meaningful comments regarding your qualifications (advisor, supervisor, etc). Please have your references complete the recommendation form (below) and provide a signed letter of recommendation on University/Company letterhead. Recommenders may send their letters directly to Bobbi Bradley via e-mail.

Please list the name of the two people who will provide letters of support for you.

1. Personal Statement

Please attach a typed statement (500 words maximum) describing the following:

1)your interest in research

2) your career goals

3) how your participation in the program will help you achieve your career goals

4) how your academic and work experience has prepared you for this program

Transcripts are required. Please include an unofficial or official transcript from all undergraduate and graduate schools you attended.

Please attach a resume or CV that includes education, work/volunteer/research experience, and achievements.
