Job Description:Independent Living Program (ILP) Case Manager

Definition

The ILP Case Manager is responsible for providing information and referrals, case management, outreach and direct services to youth in the foster care system and/or have aged out of the foster care system. This position will report to the Independent Living Program Director.

Duties

  • Conducts client intakes for ILP Department programs.
  • Provides case management services to youth who are in foster care and/or have aged out of foster care ranging in age from 16-21.
  • Profile and screenfor needs of ILP clients and provide appropriate referrals to community resources.
  • Conduct safety inspections of client’s homes and assist with developing an appropriate plan as necessary.
  • Prepare and provide reports to Courts and other relevant entities.
  • Conducts follow-up activities with ILP clients.
  • Conducts outreach at specified sites to identify youth and families in need of services.
  • Provides information and referrals for individuals referred to the program by staff from Washoe County Human Services, Juvenile Services, school district or community-based organizations as well as “walk-in” or self-referrals.
  • Supervises ILP interns and volunteers.
  • Enters updates and monitors information in client databases for case management.
  • Submits monthly reports as directed.
  • Provides input into the development of and implements ILP Department programs.
  • Achieves and understanding of the program budget; monitorsthe program budget and alerts ILP Program Director of any issues.
  • May be required to transport clients
  • Achieve an understanding of agency mission, strategic focus areas and overall goals and objectives. Contribute to the success of these areas.
  • Perform other duties as assigned.

Minimum Requirements Needed

Knowledge of

  • Family and youth relationship issues.
  • Appropriate interactions with youth and families.
  • Case Management skills
  • Community Resources

Ability to

  • Keep appointments punctually, be organized and demonstrate follow through.
  • Learn new ideas and skills by reading and with hands-on-training.
  • Express ideas clearly and concisely both orally and in writing.
  • Work with adults and youth of diverse backgrounds and beliefs.
  • Problem solve in stressful situations

Special Requirements

  • Bachelor Degree or equivalent experience in a Human Services or related field.
  • Bilingual preferred.
  • Able to pass complete background check.
  • Must be able to walk up and down stairs.
  • Must be able to lift and carry up to 25 lbs.
  • Willing to obtain needed medical screens and immunizations such at TB test, Hepatitis, etc.
  • May need to travel locally and out of state for work and training.
  • May require occasional night and weekend work.
  • Must have reliable transportation to and from work.

License or Certificate

  • Possess a valid Nevada Driver’s License and proof of insurance if utilizing a personal vehicle for company business.
