Most 2016 Graduates will be enrolled directly into the Supports Program when entering the DDD system. In order to ensure that the individual will be adequately served through the Supports Program, the Support Coordinator must conduct the following discussion with the individual/family to determine whether the graduating student can be enrolled into the Supports Program or should enter services through the Interim System at this time.

If you discover that the individual is in any of the following circumstances, please inform your Division SC Quality Assurance Specialist (Division Mentor):

  • Non-DAC
  • Enrolled in Managed Long Term Services & Supports (MLTSS) or the CCW
  • In need of Private Duty Nursing (PDN)

Question 1: Is the individual planning to attend DDD-funded camp this summer?

☐Yes

Please explain that the individual will not be able to access DDD funds for camp at this time if enrolled in the Supports Program and has the following options:

  1. The individual can choose to enroll in the Supports Program and not go to camp this summer

-OR-

  1. The individual can go to camp and enroll in the Supports Program when camp is completed (most likely in the fall)

☐No – Please continue to the next question

Question 2: Is the individual planning to utilize a Self-Directed Employee (SDE) to provide services?

☐Yes

Please explain that SDEs cannot yet be utilized in the Supports Program as the Division is awaiting a final award of the Department of Human Services Fiscal Intermediary contract to operationalize this process so the individual has the following options:

  1. The individual can choose to wait to be enrolled in the Supports Program and receive DDD services through the interim system instead

-OR-

  1. The individual can choose to be enrolled in the Supports Program with the understanding that he/she will need to access DDD services through a provider instead of SDEs until the FI is up and running

☐No – Please continue to the next section

Enroll into the Supports Program

If the answers to questions 1 & 2 were both “No” or the individual has chosen to be enrolled into the Supports Program instead of attending camp or utilizing SDEs, proceed with enrolling him/her into the Supports Program by completing the following tasks:

  • Enter/update all information in iRecord (contact info, SC and SC Supervisor assigned appropriately, etc.)
  • Review the Participant Enrollment Agreement with the individual/family and ensure understanding / answer questions as applicable
  • Scan and upload the signed Participant Enrollment Agreement to iRecord following the “Info Guide on SP Enrollment in iRecord” document
  • Be sure to upload the document through the “Plans” tab as described in the guide or you won’t successfully enroll the individual into the Supports Program
  • Proceed with plan development as usual (PCPT completion and develop/approve ISP)
  • SC and service provider collaborate to ensure all services are written into the ISP (unbundle programs to include all services provided) to ensure prior authorization for all services. It is good practice to share the ISP and Service Detail Report with the provider prior to approval to ensure information is accurate.
  • Tiers and new budgets are applied
  • Utilize the standardized rates and procedure codes provided in the “Quick Guide to Supports Program Services”
  • All services are available to individuals enrolled in the Supports Program with the exception of summer camp and those services provided by a Self-Directed Employee. Please remember to follow the approval process outlined in the “Quick Reference Guide to Service Approvals” found in Appendix F of the Supports Program Policies & Procedures Manual for those services in need of additional steps for approval.

Do not enroll into the Supports Program (use Interim System instead)

If the answers to questions 1 or 2 were yes and the individual has chosen to continue to attend camp and/or utilize SDEs instead of enrolling into the Supports Program at this time, proceed with providing services through the Interim System instead. DO NOT upload the signed Participant Enrollment Agreement at this time.

For anyone entering the DDD service system:

If a service provider cannot be located due to lack of capacity within the individual’s area, lack of ability to meet the individual’s particular needs, lack of providers for a particular service, etc., the Support Coordinator must report that information to his/her assigned Division SC Quality Assurance Specialist. The Division will track this information in order to assure that adequacy of network is addressed.
