Instructions For Requesting A Written Evaluation From Your COUNSELOR

Deadlinesto submit all materials:

September 23rdIf College application deadline in on or before 11/1

YOU are the most crucial role in this entire process! Take ownership. It is important that you meet all deadlines.

YOU are responsible for the following tasks:

  1. COMPLETEyour Brag Sheet as soon as possible. Remember that the person writing your letter of recommendation wants to write a great letter on your behalf, so it is very important that you take the time to answer questions thoroughly. The Brag Sheet must be TYPED. The template instructions are available on Naviance under the document library tab.
  1. SUBMIT requests for School Reports, Counselor Written Evaluation, and Teacher Recommendation Letters by following the instructions below.
  1. REQUEST official test scores to be sent to your college(s) or university(ies) directly from the College Board (SAT Reasoning and SAT Subject Tests) and/or the ACT.
  1. SUBMIT your application(s) to the college or university before the due date.
  1. FOLLOW UP with each college(s) or university(ies)after you apply.
  1. CHECK your email and college portal often because colleges may contact you to ensure that all required documents have been submitted properly and/or to request additional information.

Instructions for Guidance Counselor Written Evaluations

Most private colleges/universities, as well as out-of-state schools, will require a School Report or a similar form (“To Be Completed by the High School”), which is usually accompanied by a counselor written evaluation. This School Report must be completed by your Counselor. Your Counselor will also include a written evaluationusing your completed Brag Sheet and complete all required documents if he/she is asked to write a written evaluationon your behalf. He or she will also include a transcript and aMira Costa High School Profile. These forms will be completed electronically in most cases. Please carefully follow the provided instructions to avoid any confusion or mistakes in requesting that your documents be submitted electronically.

Please FOLLOW these simple instructions to request a Written Evaluation from yourCounselor:

  1. COMPLETEthe Counselor Documents Request sheet on the front of this packet.
  1. INSERT a copy of your completed Brag Sheet including the question/answer portion.
  1. REQUEST transcripts. If your school requires a paper copy, pay $5.00 for each transcript in the Student Store. If your colleges accept electronic transcripts, there is no fee and you may skip this step.
  1. SUBMITTHIS ENVELOPE DIRECTLY TO YOUR COUNSELOR IN PERSON.

Instructions For Requesting A Letter Of Recommendation From TEACHERS

Instructions for Teacher Recommendations

Most private and many out-of-state colleges and universities will require 1 or 2 teacher recommendation letters for your application. College and university admission committees read these letters to learn of your academic background, so please choose your teachers wisely. Find an appropriate time to ask your teacher for a Letter of Recommendation and please provide your teacher with ample time to write the letter. Your teacher will appreciate having at least 3 weeks to work on your letter.

Please FOLLOW these simple instructions to request a Letter of Recommendation from yourTeacher:

  1. STAPLE a completedTeacherRequest For Letter of Recommendation sheet (which is available at the end of this packet) to the front of a 9x12 envelope.
  1. INSERT a copy of yourBrag Sheet including the question/answer portion.
  1. SUBMIT the packet to your teacher by the deadlines above.

After you have submitted the above materials to your teacher, you must request the letter of recommendation through Naviance by completing the following steps:

  1. GO to the Naviance website:
  1. ENTER your username and password. Username is your first name & last name, no spaces. Password is your birthday in numeric form. Do not include 0 when entering Date of Birth, e.g. August 8th, 1998 should be entered as 8/8/1998 NOT 08/08/1998.
  1. CLICKon the Colleges tab in Family Connection, thenCLICK onColleges I’m Applying To.
  1. SCROLL to the Teacher Recommendation section and CLICKAdd/Cancel Requests.
  1. SELECT a teacherfrom the drop down menu.
  1. REPEAT for all teacher requests you want to make.
  1. CLICKUpdate Requests.

After your requests have been made, they will appear in the Recommendation Request Manager. Teachers will also receive an email notification of your request. You must complete both steps- see your teacher in person and complete the request through Naviance as stated above.
