Instructions for Use: This form is to be completed by NDOR M&R Pavement Design Section to check the Pavement Design, Final Quantities and Special Provisions in the Final Plan Set package prior to submitting them to NDOR PS&E. The NDOR LPD PC will submit a copy of the Final Plan set, final quantities, special Provisions and the Pavement Determination to NDOR M&R to conduct this review.
