Pathology Concordance

Use the omitted mamm data report to ensure pathology has been collected and entered into system on all procedures. Use this report to help you keep track of where you are in the process of getting the path and getting it to the radiologists. (Highlight with different colors as to where you are in the process: waiting for path, sent to rad, waiting for addendum….etc)

Write accession number of procedure on the top of the pathology report (will be helpful when PACSscanning the path).

Record the name of the rad that performed the procedure. (and also those other rads that may have read the screening or other add’l imaging.) (jot their initials on the back of the path for quick reference when you are putting path in rad folders)

Be sure the procedure is linked to the recommendation that it will resolve.

(Resolve the recommendation in the patient data screen—this should have been done at time of completion by the tech doing the procedure but needs to be verified for accurate statistics)

Enter pathology (this can also be done from the interventional procedures field when entering data from the interventional procedures form)

Click on “new pathology”

Pathology Findings.

Resolve recommendation.

Scan the path report to PACSscan using the procedure accession number.

In Isite identify the patient and procedure.

Double click to open the procedure.

Right click on the gray bar to the far left.

Click on add exam to folder.

Mamm Biopsy Addendums

Choose the rad that performed the procedure.

(Use the Mamm Path Follow up folders for other rads that have read screening or other add’l imaging.)

This case has now been entered in to the rad(s) folder.

Highlight the exam you just added to this folder.

Type any info that may be helpful to the rad (LOC, no calcs, mailignant, need to

phone…..)

Email the radiologist(s) that there is new pathology in their folders.

Entering Interventional procedure data.

Read the addendum the rad dictated.

Check report for concordance and new recommendation.

Mamm/Interventional procedures

Enter data in the required fields interventional procedures.

Go to Patient notes (top of field)

Enter concordance and complications info.

Go to pathology and enter path findings and resolve any appropriate

recommendations.

Enter the new recommendation from the addendum and choose appropriate

modifier.

Post Biopsy Negative: 6 mos follow up

Post Biopsy Negative: return to screening.

Post Biopsy Surg: Needs additional intervention.
