Parent & Teen Responsibilities Contract (Gr 7-HS)
Confirmation is the final sacrament of initiation for Catholics and requires a serious commitment from both parents/guardians and the teen participant. Please pray as a family while reviewing the following list before initialing each statement.
By initialing each statement, I as a parent or guardian and teen participant agree and commit to the following:
Parent Teen
______I give my consent tofully participatein the Confirmation Process.
______I will be responsible in arriving and leaving on time, every time.
______I will respect parish property, peers, priests/deacons, catechetical leaders and other volunteers involved in the parish program.
______I will attend Sunday mass every week, and learn the basic prayers of my Catholic Faith: Our Father, Sign of the Cross, Glory be, Act of Contrition
______I agree to attend the Pastor’s workshops (Parent and teen), all retreats (teen), and parent meetings to receive important info about my child.
______I will respect the privacy of other teens by not sharing what is said in small groups.
______I understand that I am an apprentice to Christ and am willing to try my best to undertake the responsibilities and behavior required to be Confirmed as a follower of Christ.
______I understand that it is my child’s choice to receive the Sacrament of Confirmation, and that it cannot be forced.
______I agree to attend at least 80% of the sessions in order to be properly prepared to receive the Sacrament(s). (Only 3 absences of any kind are permitted).
______I have read the Youth Faith Formation 2017-18 Handbook, and accept the policies and requirements.

Contrato de Responsabilidades de Padres y Adolescentes (Grdo 7-HS)
La confirmación es el sacramento final de iniciación para los católicos y requiere un compromiso de ambos padres / tutores y el participante adolescente. Por favor oren en familia mientras revisan la siguiente lista antes de iniciar cada declaración.
Al inicial cada declaración, yo, como padre o tutor y adolescente, estoy de acuerdo y me comprometo con lo siguiente:

______Doy mi consentimiento para participar plenamente en el proceso de confirmación.
______Seré responsable de llegar a tiempo y salir a la hora adecuada, cada vez.
______Respetaré la propiedad parroquial, compañeros, sacerdotes / diáconos, líderes catequéticos y otros voluntarios involucrados en el programa parroquial.
______Asistiré a la misa dominical cada semana, y aprenderé las oraciones básicas de mí Fe Católica: Padre Nuestro, Signo de la Cruz, Gloria, Acto de Contrición
______Acepto asistir a los talleres del Pastor (Padres y adolescentes), todos los retiros (adolescentes) y juntas de padres para recibir información importante sobre mi hijo.
______Respetaré la privacidad de otros adolescentes al no compartir lo que se dice en los grupos pequeños.
______Entiendo que soy un aprendiz de Cristo y estoy dispuesto a intentar lo mejor posible para asumir las responsabilidades y el comportamiento requerido para ser Confirmado como un seguidor de Cristo.
______Entiendo que es la elección de mi hijo el recibir el Sacramento de la Confirmación, y que no puede ser forzado.
______Estoy de acuerdo en asistir por lo menos al 80% de las sesiones para estar debidamente preparado para recibir el (los) Sacramento (s). (Sólo se permiten 3 ausencias de cualquier tipo).
______He leído el paquete de Formación de Fe para jóvenes 2017-18 y acepto las pólizas y los requisitos.
