Operator Safety Training

& Qualification Record

The copy of this worksheet will be used to document the training of each operator.

Each operator’s worksheet is an auditable document, and must be filed in the Operator Training Log binder in the NLCTA control room.

Notes:

·  As requirements change, an operator may need to use several versions of the worksheet. All copies shall be retained in the Operator Training Binder.

·  Qualification requirements may change with time, requiring that the area be re-appraised. The “Authorization Rescinded” box in the following worksheets will be used to remove the operator’s endorsement in the applicable areas if needed.

·  If the operator looses his or her competency or familiarity with NLCTA Operations (because of a change of employment, for example), the “Authorization Rescinded” box in the following worksheets will be used to remove the operator’s endorsement.

·  If a qualification is dropped as a necessary competency to serve as a NLCTA operator, the Safety Officer or ADSO may indicate such in the “Not Required” box.
