Hamlet Swim Club – Tennis Team Participant

RELEASE AND HOLD HARMLESS

As the parent or legal guardian of the above-identified child, I hereby release, discharge and acquit the Hamlet Swim Club, its officers, directors, coaches, agents, volunteers, members, employees, and authorized representatives from, and waive any and all rights to, all claims or demands for damages or losses the undersigned and/or my child may have against those parties, for any and all injuries suffered by my child, including those resulting from negligence, and/or the loss of or damage to any personal property owned by my child through his/her participation in swim, dive or tennis team activities sponsored by the Hamlet Swim Club.

The undersigned also acknowledges that, from time to time, coaches or other representatives of the Hamlet Swim Club may provide my child with transportation (referred to hereafter as “Club Transportation”) to swim/dive meets, tennis matches, or other activities sponsored by Hamlet Swim Club. The undersigned hereby releases, discharges and acquits the Hamlet Swim Club, its officers, directors, coaches, agents, volunteers, members, employees, and authorized representatives from, and waives any and all rights to, all claims or demands for damages or losses the undersigned and/or my child may have against those parties, for any and all injuries, including those resulting from negligence, suffered by my child and arising out of Club transportation.

The undersigned attests and verifies that my child is physically fit and capable of participating in the swim, dive or tennis team activities sponsored by the Hamlet Swim Club. I further attest and certify that I am the parent and/or legal guardian of the above-identified child and have the power and authority to execute this Release for the benefit of the Hamlet Swim Club, and agree to indemnify, save and hold harmless the Hamlet Swim Club, its officers, directors, coaches, agents, volunteers, members, employees, and authorized representatives from all liability, loss or damage which may be imposed upon these parties because of any defect in or the lack of capacity of the undersigned to so act.

Volunteer Requirements

The Hamlet Tennis Team is a completely volunteer run organization that cannot exist without parental involvement. Parents are an essential part of running our tennis team. We need 3 parents to volunteer for a home match and 1-2 parents to volunteer to be drivers for an away match.

Each parent should volunteer for ONE match. We will send an online invitation to register via signupgenius.com to either bring fresh fruits, snacks and drinks for a home match, or be a volunteer driver for an away match.

Please also consider volunteering to help in organizing the end of the year party that is tentatively scheduled this year for Friday,July 22, 2016 at 3:30 PM.
