FELINE ADOPTION CONTRACT

Please initial all numbers and sign at the bottom.

All items MUST be agreed to in order to adopt from our organization.

In consideration of adopting this animal from Nashville Cat Rescue (NCR), Adopter agrees to the following terms:

______1. Effective immediately, Adopter will provide proper and sufficient food, water, shelter, and kind and careful treatment for the cat at all times. Adopter understands that she/he is making a lifelong commitment to this cat, which may live approximately 15-20 years. Adopter agrees to provide all necessary care, including reasonable and customary medical attention.

______2. Adopter will not allow the cat outdoors unsupervised. When transporting the cat, the adopter understands that the cat should be transported in a carrier for his/her safety and protection.

______3. If, at the time of adoption, the cat has claws intact, the Adopter agrees NOT TO DECLAW the cat at any time in the future. Should it be determined by NCR that the cat has been declawed, the cat shall be returned to NCR upon notice to the Adopter. Further, the Adopter shall pay $500 per paw declawed to NCR in the form of liquidated damages, separate and apart from any other damages set forth in this contract.

______4. Adopter will take the cat to a veterinarian for examination and necessary immunizations and will procure immediate veterinary care should the cat become ill or injured, all at the Adopter’s own expense. Adopter understands that urination outside of the litter box is often associated with infection and is cause for medical attention.

______5. Adopter understands that effective upon NCR’s release of cat to Adopter that the Adopter assumes full responsibility for the health, well-being, and medical care of cat.

______6. Adopter is fully aware that NCR makes no guarantees as to the health, temperament, disposition and/or training of the animal. Adopter understands that NCR has fully disclosed all information to the best of our knowledge regarding the health, temperament, disposition and/or training of the animal.

______7. Adopter fully and completely releases NCR and its agents from any defects or illnesses the cat may have or develop and from any claim,cause of action or liability for any illness Adopter’s other animals may develop, even if said illness may have been procured from the animal adopted from NCR, and for any injury or damage to persons or property which may be caused by the cat.

______8. If Adopter adopts multiple cats from NCR at the same time, the animals will reside togetherfor their entire lives. Adopter must obtain written approval from NCR to separate the animals.

______9. I understand that NCR does NOT test every cat for feline aids and feline leukemia. If the mother cat is present, she is tested but NOT her litter. If no mother, a sibling is tested but NOT all siblings. If neither of these conditions applies, the individual cat is tested. Based on this, Adopter understandsthat his/her new pet may or may not have been directly tested and will not hold NCR liable for any false negatives.

______10. Adopter agrees to respond to NCR inquiries regarding the health/well-being of the cat.

______11. If the Adopter cannot keep the animal for any reason, the animal must be returned to NCR. Adopter agrees to set up an appointment with NCR for the animal’s return. Adopter will not give away or euthanize (“put to sleep”) the cat unless deemed necessary by a veterinarian due to medical circumstances.

______12. If, within 14 days of original adoption date, Adopter decides to discontinue adoption,Adopter may request an appointment for return of the animal with a full refund of the adoption fee. Refunds will be processed within 2-3 weeks.

I, the Adopter, have read this adoption agreement, fully understand it, and agree to be bound by its terms.
