Instructions for completing this form:
1. If you are a reselling ISV (a software vendor who resells its own packaged intellectual property), please check the box indicated.
2. You can obtain your MBS Account Number at https://mbs.microsoft.com/voice/ within PartnerSource.
3. Indicate the Licensed Software Product you intend to distribute, subject to the Terms of the SPA.
4. Refer to the SPA Guide for detailed enrollment instructions.
5. Submit this form along with your other SPA documentation to the Microsoft Regional Operating Center where you are located.
New Partners only:
6. If you plan on doing business in Europe, Middle East and Africa, please supply a local VAT number (if applicable).
7. If you plan on doing business in Australia, please provide an Australian Business Registration Number.
