Applications are invited for a post for maternity leave cover in NHS Greater Glasgow & Clyde. The post is for three months in the first instance, beginning in June 2015 and may be extended up to 10 months. You will work with adults with learning disabilities, and provide psychiatric input to multidisciplinary teams of specialist healthcare and social work professionals covering the area of North East Glasgow Community Health Partnership (CHP). There is no in-patient responsibility attached to this post. The post is based with North East Community Learning Disability Team in the Stobhill Hospital Campus and has a catchment area population of approximately 100,000. You will work alongside 11 other Consultants in Learning Disabilities Psychiatry. NHS Greater Glasgow & Clyde serves a total population of 1.2 million.

The successful applicant willhave full GMC registration and a licence to practise and be an Approved Medical Practitioner for the Mental Health (care & treatment) Scotland Act. CCT in Psychiatry of Learning Disability is preferred; applicants with CCT in other psychiatric specialties must show evidence of competence in working with adults with Learning Disability.

THE GLASGOW UNIVERSITY CENTRE FOR EXCELLENCE IN DEVELOPMENTAL DISABILITIES

1)Organisation, leadership and Management

a)Organisational Structure

NHS Greater Glasgow & Clyde is organised as follows:

  1. An Acute Services division, which provides hospital and specialist services for medical and surgical specialties;
  2. Directorates which provide some hospital and specialist services across the area; and
  3. Community Health (& Care) Partnerships (CH(C)P). CH(C)Ps are responsible for coordinating primary health care with social care, and local specialist community care (including local learning disability services).

b)General management

Learning disability services across NHSGGC are developing in line with the recommendations of ‘Promoting health, supporting inclusion’ for a tiered service. Local multi-agency multidisciplinary services (Tier 3) are organised at the level of CHP/CHCPs. A service manager (or head of learning disability) for each CHP/CHCP is responsible for general management of the local service, including the community-based element of consultant posts.

Services organised across several CH(C)Ps (Tier 4) are managed by Tier 4 Learning Disability Services.

c)Medical management

Medical staff in Learning Disability services are managed by the Clinical Director, Dr Elita Smiley. The job plan for each consultant is agreed individually, in the context of the overall requirements of the service. These include both Tier 3 duties (such as clinics, community visits and team activities) and Tier 4 duties (such as on-call, cover arrangements, inpatient work and the Epilepsy clinic).Each consultant is expected to participate in annual appraisal.

2)Services provided by NHSGGC

a)In NHSGGC, we work together with our partners and local communities to provide a full range of healthcare for 1,196,335 people living in:

d)Community services for people with learning disabilities are organised through CHP/CHCPs, which aim to provide care management for every client of the service.

e)Specialist services provided across multiple CHP/CHCPs are provided through partnership or directorate arrangements.

3)Services provided in association with NHSGGC Mental Health services

Specialist health professionals working within the Learning Disabilities service provide NHS services to adults with a learning disability throughout NHS Greater Glasgow & Clyde. In addition to sessional time from psychiatrists and clinical psychologists working with community-based learning disability teams there is a Complex Needs Service, including an out-of-hours service, and Learning Disability Epilepsy Nurse Specialists. There is also a specialist clinic for people with LD and epilepsy.A network of Autism advisers work with mental health teams and learning disability teams. In-patient services for patients living with NHSGGC have just recently undergone a period of re-design. Twelve acute assessment and treatment beds are now provided at Claythorn House which is within the Gartnavel Royal Hospital Campus in Glasgow. Sixteen community based assessment and treatment beds are provided at Blythswood House in Renfrew. Eight community based longer stay beds are provided at Netherton in Anniesland and six community based longer stay beds are provided at Waterloo Close in Kirkintilloch.

Child and Adolescent learning disabilities psychiatry services for Glasgow are provided by Dr Ama Addo in the multidisciplinary LD-CAMHS team.

The UCEDD

The UCEDD is part of the Institute for Health and Wellbeing, University of Glasgow. It was established in 2000 to maximize the health, well-being and lifestyle opportunities of individuals with developmental disabilities. To achieve this UCEDD delivers:

Active collaboration between academics, health and social care professionals, individuals with developmental disabilities and carers is central to the work of the UCEDD. The UCEDD is a partner in the Scottish Consortium for Learning Disability and has formal links with relevant international organisations.

JOB DESCRIPTION

1)Job purpose

The purpose of the post is to provide and develop a psychiatric service for adults with learning disability in the North East Glasgow area of NHS Greater Glasgow & Clyde. The post holder will work with people with learning disability and related developmental disabilities who also have mental illness or other mental health problems. Clinical activities include:

a)Assessing the mental disorders of adults with learning disability. Giving diagnosis and prognosis to patients and relatives in a way they can best understand. Assessing capacity to consent and assessing risk.

b)Supporting patients in treatment choices. Offering treatment, access to healthcare facilities, and follow-up.

c)Contributing to cover for emergencies and continuity of care.

d)Fulfilling duties required by the Mental Health (care & treatment) (Scotland) Act 2003, the Adults with Incapacity (Scotland) Act 2000, and other statutory duties requiring a psychiatrist with expertise in assessing people with developmental disabilities.

e)Providing information, advice & support to families and carers.

f)Team-working with multidisciplinary teams and independent care-providers. Giving medical leadership to the development of multi-agency working.

2)Clinical Duties

a)The role of NHS consultant in learning disabilities psychiatry is to provide psychiatric input to multidisciplinary teams of specialist healthcare professionals and staff of Local Authority social work services departments within the area covered by NHS Greater Glasgow & Clyde. Community-based work will be with the multidisciplinary learning disability services of North East Glasgow covering a population of approximately 100,000. Acute hospital liaison will be to local acute hospitals.

b)This will involve working closely with the Head of Learning Disability Services for North East Glasgow CHP and team members of the CHP learning disability services. There is one other part-time Consultant working within the North East Glasgow community learning disability team.

c)The area covered by this post may be changed through the process of job planning with the Clinical Director, and by negotiation with other consultants, to ensure equitable coverage and workloads.

d)The appointee will be expected to deliver learning disabilities psychiatric services to this population in a variety of settings including patients’ homes, NHS facilities and local authority premises.

e)The appointee will also be expected to play a significant part in developing specialist health services for people with learning disabilities in NHS Greater Glasgow & Clyde in accordance with plans agreed by the NHS Greater Glasgow & Clyde and its partnership organisations.

3)Clinical services

a) Learning Disability Team

i)Mental health services for adults with learning disability are expected to be configured using the tiered model described in the Scottish Executive report, “Promoting health, supporting inclusion”. The core clinical responsibility of the post holder will be at Tier 3, via the community-based team.

ii)Clinical activities will include regular clinics held in the area of North East Glasgow; home visits (which will be joint with other team members until any setting has been assessed as safe for a lone worker), clinical reviews/ care planning meetings, visits to local hospitals for liaison, and clinical discussions as part of individual care plans and for practice review and development.

b)Inpatient Services

This post does not include responsibility for any in-patient work. Any community patients admitted to hospital will be managed by one of the four consultants with in-patient responsibility.

Learning Disabilities Psychiatric Assessment beds associated with this post are accessed via the admissions priorities group. Eligibility for admission to each unit is based on clinical priorities across the service and therefore the patients of each consultant have equal access to inpatient facilities.

c)Out-of-hours and emergency cover

If required, the appointee may be expected to take part in one of the combined psychiatry on-call rotas for NHSGGC, at the frequency worked by other NHSGGC consultants in learning disability psychiatry, about one in thirty (subject to negotiation). Cover is reciprocal, with psychiatrists on the combined rota providing emergency psychiatric cover out-of-hours for people with learning disabilities.

d)Other clinical commitments

i)Cover for leave and to enable part-time working is provided by reciprocal agreement between the consultants in learning disabilities psychiatry in NHSGGC. The appointee will be expected to participate in these arrangements. At present, these include participating in internal arrangements to ensure continuity of availability of an Approved Medical Practitioner for the Mental Health (Care & Treatment) (Scotland) Act 2003 one session weekly.

ii)Other commitments which may include sessions, if appropriate, for special clinical interests will be agreed in discussion with the Lead Associate Medical Director for Mental Health, the Director of Human Resources and the Clinical Director for Learning Disabilities. Special clinical interests will usually be for services offered area-wide or to a larger population, and may form part of a Managed Care Network.

4)The Learning Disabilities Division Medical Staffing Profile

a)Consultant staffing

There will be 10 whole time equivalent NHS Consultants in Learning Disabilities Psychiatry in the NHSGGC service with an additional 2 academic posts attached to the UCEDD with honorary consultant status. Additionally, Consultants in Forensic Learning Disabilities Psychiatry are employed through the Directorate of Forensic Mental Health, and a Consultant in Child and Adolescent Learning Disabilities Psychiatry is employed through the Children’s Directorate.

b)Non-consultant Psychiatric Staff

Arrangements for sessional time from Specialty Doctors are currently under negotiation but are not currently available to support this post.

c)Trainee psychiatrists

There are four posts at CT2-3 across NHSGGC. Subject to educational approval, the appointee may share a trainee psychiatrist from one of the West of Scotland Core Psychiatry Training Schemes with one other Consultant colleague. Details of psychiatric training posts are subject to approval by the college tutor and by PMETB.

d) Other medical staff

The in-patient units all have primary care services provided by local GPs.

5)Administrative Support.

a)The appointee will have an office, initially based at Trust Headquarters, StobhillHospitalwhere his/her secretary will also be based.

b)A system will be in place to ensure that the postholder can work safely in the community. This will include ensuring other clinical staff attend clinics; joint visits where the situation is not known to be safe for a lone worker; and a diary and notification system to ensure that a doctor working alone is known to return to a safe location after each visit.

c)The postholder will be provided with a mobile phone (or recompensed for use of their personal mobile phone) to ensure safe communication whilst in the community.

6)Job Plan

The appointee will, by agreement with the Clinical Director for Learning Disabilities, the Lead Associate Medical Director for Mental Health, and the Director of Human Resources, have a defined job plan. The commitment of Programmed Activities is expected to be as follows:

The timing of PAs is negotiable within the framework of the national agreement on the consultant contract. Some of the Direct Clinical PAs may be replaced by clinical specialist activities, depending upon the needs of the service and the special interests of a suitable appointee.

7)Teaching

a)Undergraduate

There is the opportunity for the appointee to contribute to the University of Glasgow’s medical undergraduate curriculum.

b)Postgraduate

The appointee will be expected to participate in the Learning Disabilities Psychiatry Division’s weekly training programme, and to offer teaching to junior psychiatrists. The appointee will also be expected to participate in the postgraduate programme of the West of Scotland Higher Training Scheme in Learning Disability Psychiatry.
