between

NAME OF FAMILY

and

KIDS FIRST, INC.

Name of Family and Kids First, Inc. agree to the following:

1.Name of Family agree(s) to provide foster care services, including, but not limited to daily care and treatment, room and board, local transportation, clothing and other necessities for each child placed in the care of Name of Family. Kids First, Inc. will pay to Name of Family a fee based on the following schedule as of (INSERT DATE):

Basic $23.10 /diem/child

Moderate:$40.44 diem/child

Specialized:$51.99/diem/child

Other:______/diem/child

2.The fee shall be paid to Name of Family by Kids First, Inc. on the twentieth day of each month during the term of this Agreement.

3.Name of Family agree(s) not to accept non-relative placements for 24-hour care from any agency except Kids First, Inc.

4.Kids First, Inc. reserves the right to remove a child from Name of Family at any time if for any reason Kids First, Inc., in its sole and absolute discretion, believes that removal would be in the best interest of the child.

5.Name of Family may terminate this Agreement by giving a thirty (30) days written notice to Kids First, Inc. Kids First, Inc. may terminate this Agreement at any time by giving written notice to Name of Family.

6.Name of Family agree(s) not to discharge any foster children to anyone without Kids First, Inc.’s knowledge and consent.

7.Name of Family agree(s) to allow both announced and unannounced visits by any employee or subcontractor of Kids First, Inc. Kids First, Inc. is responsible for regular supervision of all foster homes.

8.Name of Family acknowledge(s) and agree(s) that the arrangements for all visits by a Kids First, Inc. foster child with any person other than a member of the Kids First, Inc. treatment team must be made by Kids First, Inc. Overnight visits of foster children away from the home of Name of Family must be approved in writing by Kids First, Inc.; out-of-state visits of foster children must be approved in writing by Kids First, Inc. no less than two (2) weeks prior to the date on which the visit is scheduled to commence.

9.Name of Family agree(s) to supervise (or ensure supervision by a licensed therapeutic foster parent) each foster child placed in the Name of Family home by Kids First, Inc. at all times in accordance with Kids First, Inc.’s policies and procedures.

10.Name of Family agree(s) to abide by the policy of discipline of Kids First, Inc.

11.Name of Family agree(s) to submit the following monthly reports to Kids First, Inc. on the eighth day of each month:

Daily Logs
Therapeutic Recreation Logs
Prescription and Non-Prescription Drugs Reports
Other information as requested by Kids First, Inc. including but not limited to: school reports, medical reports, dental reports, police reports, daily schedule and rules, etc.

12.Name of Family agree(s) to participate in sufficient training to keep the foster home license current. Kids First, Inc. agrees to provide training, supervision, and support as outlined in the Kids First, Inc.’spolicies and procedures.

13.Kids First, Inc. and Name of Family agree(s) to abide by all of Kids First, Inc.’spolicies and procedures as well as The Minimum Standards for Child-Placing Agencies of the Texas Department of Family and Protective Services. Kids First, Inc. is not responsible for liability resulting from acts of commission or omission resulting from violation of Kids First, Inc.’s policies by Name of Family.

14.It is agreed and understood that Name of Family shall not be deemed to be an employee of Kids First, Inc. for any purpose whatsoever.

15.Name of Family shall have no right to transfer or assign its interest in this Agreement.

16.If any provision of this Agreement shall be held to be invalid, illegal, or unenforceable, the validity, legality, and enforceability of the remaining provisions shall in no way be affected or impaired thereby.

17.Foster Family agrees to accept and abide by all terms and conditions imposed on subcontractors by the prime contract: “Residential Child Care Contract”, which can be found on file at Kids First, Inc. or accessed at

18.This agreement is in effect from the date signed (below) for a period of two (2) years or until changed in writing and signed by both parties or until terminated as indicated above. In agreement with all above, the below sign.

19.______will transport children to and from the children’s biological family visits and sibling visits and

any visit approved by Child Protective Services.
