iPad mini Initiative Policy Guidelines

By enrolling in the iPad mini Initiative you have agreed to the following initiative requirements:

§  The iPad mini is the property of Baylor College of Medicine (BCM) and will be available to you while you are enrolled.

§  If you withdraw or discontinue your education at BCM, you must return your iPad mini to Baylor College of Medicine.

§  iPad mini privileges may be rescinded prior to graduation if you violate policies and guidelines for appropriate use of the device.

§  Only one (1) 32GB (Wi-Fi only) Apple iPad mini will be made available to each eligible student and you cannot transfer or exchange the device with other initiative participants.

§  All iPad minis will be managed via BCM’s mobile device management (MDM) software. This software enables BCM to remotely locate the device and wipe its contents if it is lost or stolen.

§  You should have no expectation of privacy regarding the device or its contents. BCM may review your files and communications at any time to ensure that you are using the iPad mini appropriately.

§  You are responsible for keeping the iPad mini safe and in good condition, and for protecting it from loss, damage or theft.

§  It is not compulsory to participate in the iPad mini Initiative to be enrolled at BCM. You can successfully complete the medical education program without the use of an iPad mini.

§  You are solely responsible for maintaining, repairing, updating, securing, replacing and backing up the iPad mini as well as any expenses; including data use and download costs that may be incurred through use of the device.

§  You accept financial responsibility for the loss or theft of the device as well as the disclosure of information resulting from failure to secure and protect the data contained on the device.

§  You may not jailbreak or otherwise tamper with the iPad mini operating system. If you jailbreak the iPad mini it will void the warranty and you will be responsible for the cost of the device.

§  You must use this device in a responsible manner and in accordance with all Baylor College of Medicine policies, State and Federal law.

§  You must backup the content of the device to prevent data loss or corruption. BCM is not responsible for the loss of any data. If necessary your iPad mini may need to be factory reset. Resetting causes all programs and files on the iPad mini to be erased and returned to its original state. Synching/backing up the iPad mini regularly will allow the restoration of all data.

§  You may not take photos or video of other students, staff, faculty, patients, or visitors without their permission. The possession, forwarding, or uploading of unauthorized photos or video to any website, network storage area, or person is strictly forbidden.

§  You may not access another individual’s materials, information or files without permission.

§  You may not remove the iPad mini’s identifying labels or markings.

§  You must immediately report the loss or theft of your iPad mini to the BCM IT helpdesk at 713-798-8737 or .

Warranties, Damage, Loss, or Theft

One-year limited warranty

Every iPad mini comes with a one-year AppleCare limited warranty that covers hardware and consumable parts against defects in materials and workmanship. For full information about the warranty, go to http://www.apple.com/legal/warranty/products/ipad-english.html.

Damage to the Device

You are responsible for the cost of replacing or fixing any damage to your iPad mini that is not covered by the one-year warranty, including, but not limited to: broken screens, cracked plastic pieces, inoperability, and so on. If the cost of repairing the device exceeds the cost of purchasing a new one, you will pay the full replacement value or potentially be discontinued from the initiative. Do not disassemble or attempt to do any physical repairs to your iPad mini.

Lost or Stolen Device

BCM cannot replace lost or stolen devices. You will be required to purchase a replacement or potentially be discontinued from the initiative.

Accessories

BCM is providing you with a Smart Cover to protect your device. The purchase of any other accessory is

your responsibility.

Apps

§  BCM is supplying the iPad mini to you for educational purposes. Only BCM-endorsed apps are supported. For a list of BCM-endorsed apps, go to [link to mobility website].

§  You are free to install any app that does not violate the BCM policies. However, any unendorsed app that causes problems with the device will potentially result in the iPad mini being “wiped clean” by BCM to restore functionality. All apps, games, and music on the device must be legitimately purchased and licensed. If illegal content, software/apps or inappropriate content is discovered, the iPad mini will be reset to factory defaults.

§  BCM has no contractual or HIPAA compliance agreements for third-party cloud-based file hosting or storage services. No BCM content can be stored or sent to these services.

Security

§  Your iPad mini will automatically lock and go to sleep when you close the cover, and wake when you open it. Use the Smart Cover that was provided with your device to secure the iPad mini.

§  Never share your BCM ECA password.

§  Do not allow anyone to borrow your iPad mini.

§  Never leave your iPad mini unattended.
