A PerformanceImprovementPlan(PIP)mayormaynot takethe placeofverbal orwritten disciplinaryactionandmayormay not beusedtosupplement a verbal orwritten correctiveaction,asappropriate.

CompletingPIPForm

1.ThesupervisorwillcompletesectionsI-IV ofthePIPform.

2.ThesupervisorwillreviewthePIPwiththedepartmentmanagerandan HR representative.AnyagreeduponrecommendationswillbeincorporatedintothePIP.

MeetingwithEmployee

3.Thesupervisorwillmeet withtheemployeeina confidentialsetting and carefullyreviewallsectionsofthePIP. Thesupervisor willseekandconsidertheemployee’sinputto modifytheimprovementplan and availableresourcesections (sectionsIIIandIV)as appropriate. Thesupervisor andemployeewillestablishreasonable,mutuallyagreedupontimelines forimprovedperformance oneachexpectation.Thesupervisorwillincorporateagreeduponchangesto thePIP.

4.Thesupervisorwillprovidetheirownand obtaintheemployee’sinitial signature anddate.Seeexplanation on SectionVII oftheform.

PIPFollowUpMeetings

5.Thesupervisorwillensurethat therearefollowupmeetings scheduledduring the60-dayevaluationperiod(minimum ofone,maximumofsix,basedonthecomplexityofthePIP).Thenumberofanddatesforfollowup meetings willbeincluded in sectionVI.Thesupervisor is responsibleforensuringfollowthroughonthesemeetings.

6.If thesupervisorbelievesthePIP was unsuccessfullycompleted(or isunsureabouttheoutcome)afterthe60-dayevaluationperiod,theywillconsult with HR to make afinal decisionandtodeterminenext steps.

7.If thesupervisordeems the PIPsuccessfullycompletedafterthe 60-dayevaluationphase,thesupervisorwillschedule a6monthand12-monthpost-PIP reviewtoensure continuedsatisfactoryperformance.

ClosingthePIP

8.Thesupervisorwillmake a determination asto whetherthePIPwas un/satisfactorycompleted andcompletesectionIXoftheform.Thedeterminationthata PIPwas completedunsatisfactorilyshouldbemadeinconsultationwith HR.

9.Thesupervisorwillprovidetheir ownand obtaintheemployee’sfinal signature anddateonSectionX oftheform,indicating awarenessthat a finaldecisionhas beenmade inconsultationwith HRaboutwhetherthePIPwas completedsuccessfully.

10.UponcompletionofthePIP, theoriginal willbeplaced intheemployee’s personnelfileforfiveyears.
