Inreviewingyourperformancereport,theOfficeofCharterSchoolsisseekinganhonestandreflectiveself‐ appraisalofstrengthsandweaknessesoftheschool’scharter,withcredibleandcompellingplansforbuildingon success,maintainingmomentum,andmakingnecessarychangesforimprovementoftheschool.

 Whereappropriate,pleasebeevaluativeandmakeyourfocusoutcomesforstudents.Whendescriptiveresponses arerequested,pleaseprovidecomprehensive,yetconciseresponses.

 Pleaseplacean“X”againstthegrade(5‐1)whichmostaccuratelyreflectsyourjudgmentofoverallqualityin responsetothequestions.
