Outpatient services

Outpatient services at the Nelson are provided by St George’s University Hospital NHS Foundation Trust and Moorfields Eye Hospital NHS Foundation Trust (sub-contracted by St George’s), giving access to a wide range of consultant-led services locally to the community in Merton.

The Nelson Health Centre provides a world-class setting for the people of Merton to receive high-quality secondary and tertiary care outside of hospital.

Trauma and Orthopaedics clinics currently available are:

• Hand, wrist and elbow
• Soft tissue knee and knee replacements
• Foot and ankle
• Hip and back
• Hip andknee

Making a referral

You can refer to outpatient services using Choose and Book (excluding endoscopy – see over) or referral forms available on the Merton CCG intranet and website.

Patients with suspected cancer should not be referred to the Nelson Health Centre and should be referred through the usual 2WW channels.

Eligibility
Diagnostic services are available to all Merton CCG patients aged 16 years or over (with the exception of phlebotomy, which can see younger patients, and endoscopy, for which patients must be aged 18 or over).

All other services are available to all Merton CCG patients aged 18 years or over

Diagnostic Services

Diagnostic services are provided by St George’s University Hospital NHS Foundation Trust.

With the exception of endoscopy, the diagnostic services listed below can be requested electronically using TQuest, the St George’s Ordercomms system, which most practices now have installed. Using this service, the results are returned to the practice electronically.

• A walk-in phlebotomy service is available Monday to Friday

• Blood tests (for the SWL Pathology service) can be requested on TQuest.
• The service also accepts the St Helier Hospital DART forms, with these samples being sent to St Helier Hospital Pathology services for testing.
• The service is available for patients aged 10 and over, although children over the age of 5 and under the age of 10 can be seen if an appointment is pre-booked.

• In addition to TQuest, this service can also be requested using the referral form on the Merton CCG intranet or website
  
• The service is available for patients aged 16 and over.

• Please use the referral form on the Merton CCG intranet or website

• The service is available for patients aged 18 and over.

• Ultrasound can be requested on TQuest.
• The service is available for patients aged 16 and over.

• A walk-in X-ray service is available Monday to Friday
  8am-8pm, and Saturday mornings.
• X-Rays can be requested on TQuest.
• The service is available for patients aged 16 and over.

What you can expect as a referrer

• Patients will be seen and treated in consultant-led services with the same clinical teams that they would be seen by at St George’s Hospital.

• Diagnostic services can be requested electronically using the St George’s Ordercomms system, TQuest, and the results are returned to the practice electronically.

• Within five working days of attendance at the Nelson Health Centre, there will be written communication, to both the patient and referring clinician, of the assessment findings and the subsequent management plan.

• Any follow-up procedure, investigation or appointment which forms part of the management plan agreed with the patient should be scheduled with the patient as part of the consultation, or immediately following it.

• If a Specialist Service recommends a new or urgent (needed within 14 days) medication for the patient then they will be provided with a prescription for four weeks’ supply.

• All medication is prescribed on FP10.

• If the Nelson Health Centre rejects a referral as being outside of the scope of the services offered there then this will be communicated to the referring clinician within two working days of receipt of referral.

Transport advice for patients

Please note that information leaflets aimed specifically at patients can be ordered from Merton CCG.

• The centre is a 5 minute walk from Wimbledon Chase train station and a 10 minute walk from Merton Park tram stop.
• The centre is easily accessible by bus from Wimbledon, Sutton, Morden, Tooting and Mitcham; buses 152, 163, 164, K5 stop directly outside the centre.
• There is parking on site; car parking charges are lower than at local hospitals.
