Hoisting In-House Training Providers Guidelines for Registration

REQUIREMENTS FORIN-HOUSE TRAINING APPROVAL

FOR CONVENTIONAL, HYDRAULIC AND TOWER CRANES

These are the In-House Training and Refresher Course requirements for exemption from the New Mexico Hoisting Operators Safety Act. The Hoisting Operators Licensure Examining Council must approve your course and instructor(s). The following information should be sent to the Hoisting Program Administrator:

  1. INSTRUCTOR QUALIFICATIONS:
  • Provide name(s) of instructor(s) that will be teaching the course.
  • For each instructor, include information and documentation that shows knowledge and certification as instructor(s). (e.g.: copies of certificates from the Occupations Safety and Health Administration (OSHA), Mine Safety and Health Administration(MSHA) or a CERTIFIED PROFESSIONAL TEACHING LICENSE).
  • List their experience as hoisting operators(alsoinclude a copy of hoisting Operator’s license if applicable).
  • Provide a list AND documentation of the training courses attended by the instructor(s).
  • Include copies of certificates(s) of training course(s) attended. (e.g. “TRAIN THE TRAINER” course).
  1. POINT OF CONTACT:
  • Provide a letter with name(s), address(s) and phone number(s) of person(s) who will serve as point of contact. NOTE:Any changes must be reported to the Department within 30 days.
  1. STATEMENT OF COMPLIANCE:
  • Send a notarized letter stating that your Initial In-House (16-hour) Training Program complies with ANSI B30.5a-1995.
  • Send a notarized letter stating that your Refresher Course (8-hour) complies with the most current ASME B30 series.
  1. INDUSTRY RECOGNIZED:
  • Explain how your training course is an “industry recognized training course” (include a letter on Company letterhead which may be signed by either the President or Vice-President of the Company).
  1. COPY OF LOAD CHART:
  • Provide a copy of the load chart exam along with the answer sheet.
  1. IN-HOUSE TRAINING PROGRAM (16 HOURS REQUIRED):
  • Provide a course outline for all of the subjects.
  • Provide an itinerary for each of the above outline subjects (not verbatim, but a breakdown of what will be covered. E.g. certain readings, safety awareness, hand signals, American National Standards Institute(ANSI), American Society of Mechanical Engineers (ASME) standards, Hoisting laws, trainings, etc.) NOTE: YOUR ITINERARY MUST INCLUDE THE NEW MEXICO HOISTING OPERATORS SAFETY ACT AND RULES.
  • State the amount of time to be spent on each subject. (For example the time for ANSI and ASME Standards, hoisting laws, training, etc.)
  • If your training program has an examination, please provide a copy along with the answer sheet. In addition, provide a notarized letter stating that your exam will rotate between 50 and 100 questions.
  1. REFRESHER COURSE (8 HOURS REQUIRED):
  • Provide a course outline for all subjects.
  • Provide an itinerary for each of the above outline subjects (not verbatim, but a breakdown of what will be covered. E.g. certain readings, safety awareness, hand signals, ANSI, ASME standards, Hoisting laws, trainings, etc.) NOTE: YOUR ITINERARY MUST INCLUDE THE NEW MEXICO HOISTING OPERATORS SAFETY ACT AND RULES.
  • State the amount of time to be spent on each subject. (For example the time for ANSI and ASME Standards, hoisting laws, training, etc.)
  • If your training program has an examination, please provide a copy along with the answer sheet. In addition, provide a notarized letter stating that your exam will rotate between 50 and 100 questions.
  1. WALLET CARD:
  • Send a sample of the wallet card that will be issued including the information required by Hoisting Rule 8.2.6.

Verbiage with information required by Section 60.-15-4(B) NMSA 1978, which states: “A person who has successfully completed anin-house training course approved by the hoisting operator’s licensure examining council may operate hoisting equipment without a license as required by Subsection A of this section for a period of one year after successful completion of that course. One year after successful completion of an in-house training course approved by the council, a person must be licensed pursuant to Section 60-15-7 NMSA 1978, except that the requirement for passing a written examination pursuant to that section shall be waived.

  1. LIST OF CERTIFICATE HOLDERS TO DEPARTMENT:
  • You must furnish in writing to the Department a list of exemption holders and the number and date of issuance of each certificate within thirty (30) days of the completion of training. You must also furnish immediately in writing to the Department any additions to or deletions from the list. Your list must include the name, address and social security number of the exemption holder. Send sample documentation of what your Initial In-House Training Program will be submitting to the Department in order to comply. NOTE: A SAMPLE OF THE REQUIREMENTS IS PROVIDED FOR YOUR CONVENIENCE. PLEASE FEEL FREE TO UTILIZE THIS FORM.
  1. Submit a Law and Safety Exam:
  • Review the New Mexico Hoisting Act and its rules. After review, create ten (10) questions and submit as your Law and Safety Exam.
  1. PER NEW MEXICO RULE 16.43.2.24 (k):

Any person or organization offering in-house training and wishing approval of its course shall submit a $300 application fee.Upon approval, a person or organization shall submit a certification fee of $400.

Any person or organization wishing to renew its certification shall submit a $300 renewal fee.

An employer offering in-house training and wishing approval of its course shall submit a $150 application fee; upon approval, the employer shall submit a certification fee of $300. Any employer wishing to renew its in-house training certification shall submit a $200 renewal fee.
