APPENDIX A

• The over 65 population in Broxbourne district is 14,900, representing 16% of the total population of the district and 9% of the total over 65 population in Hertfordshire
• 10% of older people in Broxbourne live in areas where people experience deprivation, representing 21%of the older population in Broxbourne
• The rate of emergency admissions for falls in over 65s in Broxbourne in 2010/11 was 254.2 admissions per 100,000 population aged 65+, which is slightly higher than the Herts average of 248 per 100,000
• Only 71.9% of residents in Broxbourne agreed that their local area is a place where people from different backgrounds get on well together, which is the lowest in the county
• Only 19.6% of residents in Broxbourne had given unpaid help to any group(s), club(s) or organisation(s) at least once a month over the previous 12 months (Herts average 25.27%). This is the lowest in the county

• Broxbourne Borough Council do not currently cite carers as a priority group in any strategic plans and do not have any projects currently running to support carers.
• Residents of Broxbourne are likely to use Chase Farm/Barnet hospitals
• Countywide organisations which have carers support services available to people in this district include: Age UK, Carers in Hertfordshire, Alzheimers Society, CAB, Hertfordshire Crossroads, Stroke Association, and Turning Point.
• Health and wellbeing services currently commissioned by the Community Wellbeing Team that are specific to this district include Alzheimer’s Society Dementia cafés. The Hoddesdon ‘Singing for the Brain’ sessions are the most popular in the district. Residents of the district can also access services provided by HPFT for people with young onset dementia and their carers, an Age UK Italian Outreach service and Age UK 10-3 clubs.

• Dacorum has the highest population of 65+ in the county - 22,700 people (13% of over 65 population for Hertfordshire). Proportionately this represents 16% of the total population of the district, as Dacorum has the largest population of any district tin the county
• It’s population of 65+ is projected to increase by 22% by 2020, which is higher than the Hertfordshire average of 21%
• Dacorum has the highest proportion (14%) of retired people living in areas where older people experience deprivation. This represents 19% of the older population of Dacorum

• Hemel Hempstead hospital (West Hertfordshire Hospitals NHS Trust) is located within the district, with a 24/7 urgent care centre and local healthcare facilities
• Breakaway for Carers, a service which enables relatives and friends looking after dependent adults to have a break by arranging for a volunteer to take over for a few hours in the person's home., is available in Dacorum
• Carers did feature in the LSP sub-group for Dacorum, but this group has since been disbanded and replaced by ‘Destination Dacorum’
• Countywide organisations which have carers support services available to people in this district include: Age UK, Carers in Hertfordshire, Alzheimers Society, CAB, Hertfordshire Crossroads, Stroke Association, and Turning Point.
• Health and wellbeing services forcarers currently commissioned by the Community Wellbeing Team that are specific to this district and may support carers include Herts Mind Network, which provides a range of recovery based services across Hertfordshire for these experiencing mental health problems and their carers, Age Concern Dacorum, Heather Club and Mencap Dacorum.

• East Herts has the third highest population of 65+ in the county - 21,000 people (12% of over 65 population for Hertfordshire) representing 15% of the total population of the district.
• East Hertfordshire's population 65+ is projected to increase by 30% by 2020 (this is the highest increase in the county) and will make up 19%, of the total district population
• East Herts has the second lowest proportion (8%) of retired people living in areas where older people experience deprivation. This represents 11% of the older population of East Herts
• The rate of emergency admissions for falls in over 65s in East Hertfordshire 2010/11 was 232.2 admissions per 100,000 population aged 65 plus, which is lower than the Herts average (248 admissions per 100,000)[2].

• HertfordCountyHospital (East and North NHS Trust) is based in Hertford, and provides outpatient, ante-natal/post-natal, diagnostic and therapeutic services.
• Carers in Hertfordshire report that carers in East Hertfordshire generally feel less well served, in terms of carer support services, than in other parts of the county. Carers in Hertfordshire co-ordinate a six-monthly ‘Carer network meeting’ for East Herts, for providers in the district to share information about services being provided.
• Breakaway for Carers is currently recruiting volunteers in Ware, with the aim of expanding provision in the area
• In the Everyone Matters – A Sustainable Community Strategy for East Herts carers are not specifically mentioned but it is implicit in the strategy to support carers. Carers in Hertfordshire are represented on their HWb sub-group
• Countywide organisations which have carers support services available to people in this district include: Age UK, Carers in Hertfordshire, Alzheimers Society, CAB, Hertfordshire Crossroads, Stroke Association, and Turning Point.
• Health and wellbeing services for carers currently commissioned by the Community Wellbeing Team that are specific to this district include Age Concern Bishop’s Stortford The Apton Centre, Hailey Centre (Sawbridgeworth) and Guideposts Trust

• Hertsmere District has a population of 65+ of 16,100 people (9% of over 65 population for Hertfordshire) representing 16% of the total population of the district.
• Hertsmere's population 65+ is projected to increase by 24% by 2020 (lthis is the third highest increase in the county) and will make up 18%, of the total district population
• Hertsmere has one of the highest population of non-white retired people in the county - 1,100 people (12.8% of non white retired people in the county) representing 6% of the total population of retired people the district (Herts average 4%).
• 12% of older people in Hertsmere live in areas where older people experience deprivation live. This represents 23% of the older population of Hertsmere.

• A number of GP surgeries in Hertmere took part in the HCL (Hertfordshire Commissioning Ltd) Innovations Project, wherein Carer Champions promoted onward referral to Carers in Hertfordshire once carers had been identified by the surgery. The GP surgeries who took part continue to have higher rates of referral to Carers in Hertfordshire than other surgeries. Partly as a result of this project, carers remain a high priority for Herts Valleys CCG.
• Breakaway for Carers is currently recruiting volunteers in Hertsmere, with the aim of expanding provision into the area.
• The new CareTrack system will be piloted with the Red House CCG practices.
• Countywide organisations which have carers support services available to people in this district include: Age UK, Carers in Hertfordshire, Alzheimers Society, CAB, Hertfordshire Crossroads, Stroke Association, and Turning Point.
• Health and wellbeing services currently commissioned by the Community Wellbeing Team that are specific to this district and which may support carers include lunch clubs (Sixty Plus, Potters Bar, and Bushey District Day Centre) and Jewish Care.
• Residents are likely to use BarnetHospital, Chase Farm Hospital (Barnet and Chase Farm NHS Trust) and PottersBarCommunityHospital (NHS Hertfordshire)
• Hertsmere Home from Hospital Project (British Red Cross) provides short term help to people from Hertsmere discharged from these hospitals.

• North Herts has the second highest population of 65+ in the county - 21,300 people (12% of over 65 population for Hertfordshire) representing 17% of the total population of the district.
• North Hertfordshire's population 65+ is projected to increase by 28% by 2020 (this is the second highest increase in the county) and will make up 20%, of the total district population
• North Hertfordshire has a population of 1,000 non-white retired people (11.6% of non white retired people in the county) representing 4% of the total population (Herts average 4%)
• North Herts has one of the second highest proportions (12%) of retired people living in areas where older people experience deprivation. This represents 17% of the older population of North Herts.
• The rate of emergency admissions for falls in over 65s in North Hertfordshire 2010/11 was 302.3 admissions per 100,000 population aged 65 plus, which is the second highest in the county.(Herts average 248 admissions per 100,000)

• Countywide organisations which have carers support services available to people in this district include: Age UK, Carers in Hertfordshire, Alzheimers Society, CAB, Hertfordshire Crossroads, Stroke Association, and Turning Point.
• Health and wellbeing services currently commissioned by the Community Wellbeing Team that are specific to this district and which may support carers include Royston Mencap Tuesday Club, North Herts African Caribbean lunch club, Italian Elders Association, Letchworth MIND, NorthHertsCollege summer scheme and Reynolds Day Care.
• LEOS (St John of God Care Services) provides leisure activities for people with learning disabilities and respite for their carers in North Herts.
• No specific mention of carers in North Herts District Council strategic plans, but support for carers is implicit through a general commitment to equalities. The Council grant funds activities which demonstrate a benefit for carers and supports sports and leisure activities which offer concessions for carer
• There are a number of active local partnerships, including the local health and wellbeing partnership and a 50+ network group.

• St. Albans District has a population of 65+ of 20,600 people (12% of over 65 population for Hertfordshire) representing 15% of the total population of the district.
• St Albans population 65+ is projected to increase by 17% by 2020 (lower than Hertfordshire average of 21%) and will make up 16%, of the total district population
• St Albans has one of the lowest proportions (6%) of retired people living in areas where older people experience deprivation. This represents 9% of the older population of St Albans.
• 29.4% of residents in St Albans had given unpaid help to any group(s), club(s) or organisation(s) at least once a month over the previous 12 months (Herts average 25.27%).This is the second highest in the county

• St AlbansCityHospital (West Hertfordshire Hospitals NHS Trust) is a local general hospital offering planned surgery and a minor injuries unit.
• Breakaway for Carers is currently available in St Albans City & District
• Crossroads report gaps in carer networks in the St Albans area
• Countywide organisations which have carers support services available to people in this district include: Age UK, Carers in Hertfordshire, Alzheimers Society, CAB, Hertfordshire Crossroads, Stroke Association, and Turning Point.
• Health and wellbeing services currently commissioned by the Community Wellbeing Team that are specific to this district and which may support carers include St Albans Mencap, Harpenden Mencap, Daylight Club and various recreational and educational activities for LD groups (Special Olympics; Passport to Leisure).
• Local partnerships include an active health and wellbeing partnership, St Albans Partner and Provider Forum and The Voice, a service user/carer forum led by users and carers.

• Stevenage has the second lowest population of 65+ in the county – 11,700 people (7% of the over 65 population for Hertfordshire), representing 14%of the total population of the district.
• It has the lowest population of non-white retired people in the county
• Stevenage has the second highest proportion in the county (12%) of retired people living in areas where older people experience deprivation. 32% of older people in Stevenage live in areas where older people experience deprivation
• The rate of emergency admissions for falls in over 65s in Stevenage in 2010/11 was 305 admissions per 100,000 population aged 65+, the highest in the county (Herts average was 248 per 100,000)

• Stevenage has an active Health and Wellbeing Partnership
• Lister hospital (East and North NHS Trust) is located in Stevenage and has an emergency department, making it a possible site for the carer friendly hospital pilot. Lister hospital has also been allocated CQUIN funds to support older carers and carers of people with a learning disability
• Countywide organisations which have carers support services available to people in this district include: Age UK, Carers in Hertfordshire, Alzheimers Society, CAB, Hertfordshire Crossroads, Stroke Association, and Turning Point.
• Health and wellbeing services currently commissioned by the Community Wellbeing Team that are specific to this district include: Douglas Drive Day Centre, Bidwell Day Care, Crossroads Day Centre and GEMMS.
• The Chair of East & North Herts CCG has a practice in Stevenage. The GP Champion for the East of England (Supporting Carers in General Practice Programme) reports that practices in East and North Herts CCG are less engaged with the carers agenda than those in HertsValleys. The GP Champion for the East of England used to be part of a practice in Stevenage.

• Three Rivers has one of the highest population of non-white retired people in the county - 1,100 people (12.8% of non white retired people in the county) representing 6% of the total population of retired people the district (Herts average 4%).
• Three Rivers has one of the lowest proportions (6%) of retired people living in areas where older people experience deprivation. This represents 13% of the older population of Three Rivers.
• Three Rivers District has a population of 65+ of 15,000 people (9% of over 65 population for Hertfordshire) representing 17% of the total population of the district.
• The rate of emergency admissions for falls in over 65s in Three Rivers 2010/11 was 221.4 admissions per 100,000 population aged 65 plus, which is the lowest in the county (Herts average 248 admissions per 100,000)

• Countywide organisations which have carers support services available to people in this district include: Age UK, Carers in Hertfordshire, Alzheimers Society, CAB, Hertfordshire Crossroads, Stroke Association, and Turning Point.
• Health and wellbeing services currently commissioned by the Community Wellbeing Team that are specific to this district and which may support carers include Watford African and Caribbean Association (WACA), who provide carer support services in Three Rivers, Watford Mencap, Turning Point (who provide short breaks for carers of people with Young Onset Dementia in Three Rivers and Watford only) and Watford Asian Community Care.
• There is a range of partnership activity in the district, including the Watford & Three Rivers Health Partnership and a Poverty, Employment and Prosperity Action Group, with representation from organisations linked to carers.

• Watford has the lowest population of 65+ in the county - 11,100 people (6% of over 65 population for Hertfordshire) representing 13% of the total population of the district
• Watford has one of the highest population of non-white retired people in the county - 1,100 people (12.8% of non white retired people in the county) representing 9% of the total population of retired people the district (Herts average 4%).
• 11% of older people in Watford live in areas where older people experience deprivation live. This represents 30% of the older population of Watford.
• Watford has the highest population (13%) of Non-white people in the county - 17,7000 people, representing 21% of the total district population (increase from 14.05% in 2001), significantly higher than the Herts average (12%)
• The rate of emergency admissions for falls in over 65s in Watford 2010/11 was 277.3 admissions per 100,000 population aged 65 plus, which is the third highest in the county.(Herts average 248 admissions per 100,000)

• Countywide organisations which have carers support services available to people in this district include: Age UK, Carers in Hertfordshire, Alzheimers Society, CAB, Hertfordshire Crossroads, Stroke Association, and Turning Point.
• Health and wellbeing services currently commissioned by the Community Wellbeing Team that are specific to this district and which may support carers include DRUM (day opportunities, information and support for adults physical or sensory impairment), Watford African and Caribbean Association (WACA), Watford Asian Community Care, West Watford Community Association, Herts Mind Network,
• Close proximity to hospital with an A&E department (Watford General) – a key trigger point in the caring journey.
• Services for BME carers in Watford include Watford African and Caribbean Association (WACA), who provide carer support services. This is a reflection of the fact that Watford has the highest levels of non-white people in the county.
• Turning Point provide short breaks for carers of people with Young Onset Dementia in ThreeRivers and Watford only.
• Breakaway for Carers is currently recruiting in Watford
• Carers are not cited in the corporate plan of the district council and are not an explicit health priority. However, local partnership work includes the Watford and ThreeRivers health and wellbeing partnership and Watford Together

• Welwyn Hatfield District has a population of 65+ of 17,100 people (10% of over 65 population for Hertfordshire) representing 15% of the total population of the district.
• Welwyn Hatfield's population 65+ is projected to increase by 16% by 2020 (lower than Hertfordshire average of 21%) and will make up 15%, of the total district population
• Welwyn Hatfield has a population of 700 non-white retired people (8% of non white retired people in the county) representing 4% of the total population (Herts average 4%)
• 9% of older people in Welwyn Hatfield live in areas where older people experience deprivation live. This represents 15% of the older population of Welwyn Hatfield.
• The rate of emergency admissions for falls in over 65s in Welwyn Hatfield 2010/11 was 222.3 admissions per 100,000 population aged 65 plus, which is the second lowest in the county (Herts average 248 admissions per 100,000)

• QueenElizabethIIHospital (East and North Herts NHS Trust) is located in Welwyn Garden City and has an A&E service in the day and treats minor injuries over night.
• The GP Champion for carers for the East of England is based in Welwyn Garden City.
• Carers are not named as a priority group in any district council strategic plans and are only mentioned in terms of the concession available to carers for Council Tax
• Countywide organisations which have carers support services available to people in this district include: Age UK, Carers in Hertfordshire, Alzheimers Society, CAB, Hertfordshire Crossroads, Stroke Association, and Turning Point.
• Health and wellbeing services currently commissioned by the Community Wellbeing Team that are specific to this district and which may support carers include day services for older people (Cuffley Day Centre; Friendship House; Jimmy Mac’s)
• Other health and wellbeing services in the district include YMCA Space Centres, Hatfield Space Centre and Hornbeams Day Service
