Job Title:CareManager

Responsible to: Registered Manager

Accountable to:Director

Responsibilities

  • To provide case management and anticipatory care for complex needs
  • Continuous 24 hour responsibility in partnership with the care team for leading and coordinatingthe clinical, operational, managerial and educational requirements.
  • The post holder will be responsible for effective resource management within the community care team.
  • To actively be involvedin personal and professional development of all specialist community staff within their team including mandatory training
  • As a member of the care team participates in establishing a good working environment in which all patients / clients receive a high standard of person centred carewithin the company in collaboration with the Registered Manager and Lead Nurse. .
  • Accountable for own professional actions

Main Responsibilities

Clinical

1.1Ensures the companymaintains a safe working environment in which all patients / clients receive highstandards of individualized carewhilst maintaining their privacy and dignity

1.2Liaises with the multidisciplinary team within the company, the hospital and primary care trusts, to ensure good patient outcomes

1.3To be competent in extended specialist skills

1.4Provide case management to identified case load including assessment of need, care planning and acting as a centre point of co-ordination

1.5As part of Bridges Quality Assurance, assuring that care is maintained to a high standard. Regular reviews according to the needs of the patient / client. Formally documented in accordance with CQC requirements.

1.6Assessing the need for and requesting appropriate aid and equipment and reporting any faults

1.7Recognise, react to and manage appropriately emergencies relating to this speciality

1.8To act as an expert resource for specialist carers to assess and manage clinical situations

1.9Maintain high standards of delivery of care reflecting evidence based practice

1.10To act as a resource to staff to ensure patients have adequate information/health advice.

1.11Knowledge and application of Bridges Infection control policy in all areas of work.

1.12Acts in accordance with the Bridges manual handling policy, minimizing risks to patients and staff.

Managerial

2.1To be aware of the ongoing organisational structure and culture and the implications this has on the clinical area

2.2To work with the Director to acquire new contracts and developments.

2.3When co-ordinating management of the department delegates duties and care appropriately whilst maintaining overall responsibility

2.4Initiating and participating in case conference and reviews within the multidisciplinary team

2.5To work with Bridges Management Team in the acquisition of property to develop into the future planned discharge unit.

2.6Management and supervision of Care Team

2.7Ensure staff are competent to provide a high standard of care in accordance with CQC requirements.

2.8To actively be involved in the cascading of clinical and relevant health information so that staff are fully informed and up to date.

2.9Acts under the direction of the Director

2.10To action all untoward incidents within the department and report appropriately in accordance with Bridges policies and CQC requirements.

2.11Ensures effective 24hr off duty rotas are maintained using manpower efficiently anticipating staff shortages and acting appropriately.

2.12To be responsible for the assessment and subsequent ordering of bank and agency staff within the company, including being an authorised signatory for temporary staff.

2.13Assists with the maintenance of necessary equipment and ordering of supplies as required ensuring a safe environment.

2.14 Assesses the long and short term staffing and resource needs of the department and takes appropriate action.

2.15To participate in recruitment and retention of care staff within the home care department

2.16 To ensure that the ongoing appraisal / supervisionprogram is maintained and all staffhas personal development plans.

2.17 Facilitate any measures to help achieve department targets and disseminate all appropriate information to all members of the nursing team

2.18 Is confident of own role in aMajor Incident Procedure and ensures all managed staff are aware of their role.

2.19To liaise with staff within the community and nursing department, to promote good relations and care practice between the areas.

2.20Promotes effective risk management strategies in accordance with Bridges Healthand Safety Policy and CQC requirements.

2.21Works to meet the objectives of the company.

2.22 To ensure all that the company patient/staff database is maintained contemporaneously. All Staff to be compliant in submitting and maintaining input so information is relevant and current. The system used is Tagtronics. To ensure that all staff are compliant in using Tagtronics correctly.

Professional

3.1 Contribute to service developments within the company

3.2 Ensure all staff adheres to allcompany and departmental policies and CQC requirements.

3.3 Have an awareness of evidence/research based practise.

3.4 Undergo individual performance review on a regular basis

3.5 Maintains own professional portfolio

3.6Act in accordance with Bridges Code of Conduct as well as standards governed by CQC and be accountable for his/her actions at all times.

Staff Development & Education

4.1 Keeps up to date both professionally and educationally and meets the criteria for the CQCregulations.

4.2 Actively participates in development programme and staff meetings.

4.3 Ensures all staff facilitatelearners whilst in the company

4.4 Facilitates the mentorship of staff.

4.5 Participates in orientation of all new members of staff.

4.6 Participate in clinical audit programmes or research projects as directed

Communication

5.1 Take an active role in maintaining effective communication with all

disciplines, patients and relatives that meets individual needs

5.2Deal with complaints in a calm and courteous manner, ensuring that

wherever possible complaints are dealt with efficiently and satisfactorily

and resolved locally

5.3 Assist in maintaining accurate data collection using appropriate systems

IncludingIntranet.

5.4 Ensure that all documentation is completed accurately, added onto Tagtronics and is legible and falls

within the Bridges Record Keeping Guidelines.

5.6 Able to communicate effectively in demanding and challenging situations

5.7 Works as part of the multidisciplinary team in supporting colleagues,

patients and their friends and relatives, during the communication of sudden and unexpected news.

5.8 To have structure meeting with Health Care team to process strategic

direction and to cascade information to the departments.

Statement on Clinical Governance

The primary focus of Clinical Governance is the delivery of high quality care to patients / cliwnts. At service level, clinical teams must analyse and assess the quality of the services they provide and continually seek improvement through clinical effectiveness, clinical audit and clinical risk management. These activities may also be carried out by an individual or by multi disciplinary teams. All clinicians have a responsibility to participate in this process, to share their concerns and to maintain and develop their own skills through continuing professional development.

Training and Development

Through appraisal and supervision training will be identified and be tailored to the individuals needs. This will also be inline with the companies objectives staff are encouraged at all times to enhance their skills and knowledge within their field.
