Intro Script – Initial Approach to Community Member

Hi,I’m (e.g.,Ruby) from (e.g., the Roosevelt CountyPreventionCoalition)here in (Portales). We’re conductingasurveyto find out whatcommunitymembersknow aboutalcohol and drugprevention efforts and tohelp identifyprevention needsin the community. It takesabout 15-20 minutes to complete and as an incentive, we’regivingout( ).Canyou helpus today?

important forus to understand theneeds of our communityfor appropriateprogramming.”

IfYES:Do you live hereinNewMexico? (Weask that people know thecommunitytoparticipate. If theylive outsideyourcounty, ask them to think abouttheir NM countyastheyanswer thequestions).

IfNO – “Ok, Thanks foryour time”

Well,let me tellyou alittlemore about this survey.Itasks questions about what’s happeningin our communityrelated to drinking, drinkingand driving, prescription and otherdruguse, and mental health services. Thesurveyhelps us identifythe prevention needs in our communityand address thoseneeds. It isan anonymous surveythat takes about 15 minutes. We arecollectingthis

surveyfaceto faceand via Facebook. Have you taken the survey already this year?

Script for explaining the survey and CONSENTING Person

“Great! Here’s thesurvey. (Provide a surveyonatablet orclip board with a pencilor pen.)AsI mentioned, thesurveyisaboutdrugand alcohol useinour communityandhas some questions about mental health, gambling, childhood trauma and access to services. This first pageexplainsyourrights when taking the survey. Mostimportantly you should know that this surveyiscompletelyvoluntary,you mayskip anyquestionsyou don’t want to answer,andyourresponses areanonymous, so do not put your name anywhereon the survey. Ifyou havequestionsasyou takethe survey, pleasefeel freeto askme andI’ll do mybest to answer them. Finally, ifyoufindasyou start totakethe survey, thatyou’vealreadytaken itrecently, pleaselet meknow.”
