On [date], we discussed that your employment with the Company is terminated effective [date] as a result of [specify reason for termination].

As I explained, your employment is being terminated due to [succinctly specify reason for termination in three words or less].

This letter contains important information, please read it carefully.

Final Paycheck. You will receive your final paycheck [indicate when the employee will or did receive his or her final paycheck], which will include your compensation, and accrued and unused vacation, through the effective date of your termination. Based on our records, you will be paid the gross amount of $[amount] (minus applicable tax withholdings), which represents all salary and wages due ($[amount]), and [number] days of accrued but unused vacation, holiday, and sick time ($[amount]), through the effective date of your termination. If you feel the compensation, or accrued, but unused vacation and sick time, are not accurate, please notify me immediately.

Employee Benefits. Your Company health insurance coverage will terminate as of [date]. However, you may be eligible to continue health insurance coverage under the Comprehensive Omnibus Reconciliation Act of 1986 (COBRA) or state insurance laws. In addition, you may be able to convert your group life insurance to an individual life insurance policy. The Company’s benefits department will provide detailed information about COBRA coverage [and other applicable information about your benefits, including your retirement benefits under the Company’s [identify the company’s retirement plan],] in a separate letter.
