BACKGROUND

The Expert Patient Programme (EPP) is a self-management programme for people living with a long-term condition. The aim is to support people by increasing their confidence, improving their quality of life and helping them to manage their condition more effectively.
An expert patient is considered to be someone who:
  • Feels confident and in control of their life
  • Aims to manage their condition and its treatment in conjunction with healthcare professionals
  • Communicates effectively with professionals and is willing to share responsibility for treatment
  • Is realistic about how their condition affects them and their family
  • Uses their skills and knowledge to lead a full life
A self-management course is designed to assist those with a long-term condition to be an expert patient and includes the topics referred to in the Service Description below.
Each course session is run by volunteer tutors who have been fully trained to provide a self-management course.

KEY RESPONSIBLITIES

The post holder has responsibility for the line management of volunteer tutors to include: recruitment, organisation of training, ongoing support and paperwork pertaining to the role.
The post holder has responsibility for the co-ordination of recruitment of volunteers and tutors.
The post holder will need to be able to accumulate and analyse data for performance management and evaluation purposes.
1) To be the main point of contact within the organisation for enquiries about the programme.
2) Recruit, interview, support and manage volunteer tutors through their training, accreditation and beyond.
3) Ensure volunteer tutors are trained, accredited and supervised according to identified good practice.
4) Develop and co-ordinate a schedule of community courses.
5) Develop and maintain a database of information for volunteers and provide regular reports/analysis of volunteer activity as required by the Stanford Licence agreement;maintain any other monitoring requirements thatmay be required as part of the project.
6) Monitor and evaluate programme delivery outputs and outcomes.
7) Keep up to date with the relevant policy, legislation, guidance and professional developments on a national and local level and contribute to the effective application of new policy initiatives.
8) Develop and maintain effective policies and strategies for the management of volunteer tutors in line with national best practice.

PERSON SPECIFICATION

Experience
  1. To be able to demonstrate line management of severalstaff/volunteers/consultants
  2. Experience of managing and coordinating a project from inception.
  3. Budget management including resources for different courses and managing multiple projects and resources for different clients.
  4. Design, delivery and monitoring of community-based, adult learning or other types of training activities.

Understanding
  1. The relationship between health, inequalities and relationships between people in communities.
  2. Behaviour change, health improvement and ‘self-management’.
  3. Participation and social change.

Capabilities and skill
  1. Self-organised and capable to lead by example, supporting a team of people to operate within structured systems and processes.
  2. Clear and skilled communicator in the context of diverse audiences, both verbally and in writing. This includes the ability to be effective one-to-one, through meetings, presenting to groups and at public events as well as the ability to listen to different perspectives and appreciate different ways of thinking.
  3. Ability to think creatively, systemically and critically about improvement in the context of health, community, development and social change. Ability to handle quantitative, qualitative and creative data and information relating to activity monitoring and impact evaluation.
  4. Able to use IT proactively and effectively, including outlook, spreadsheets, excel, publisher and how to work with social media.
  5. Be proactive and solution focused.

  1. To undertake any other duties commensurate with the post as the programme develops, as required by the Deputy Chief Executive Officer.
  2. Must hold a full driving licence.
  3. A DBS will be required for this post.
  4. Be prepared to travel around the County of Derbyshire.
  5. Homeworking is considered for this post with a requirement to attend the office in Swadlincote when required.
