PRODUCT WARRANTY PROGRAM CUSTOMER REGISTRATION

TERMS & CONDITIONS

COVERED COMPONENTS INCLUDE:

ENGINE - All internally lubricated parts of engine, including: pistons, piston rings, piston pins, crankshaft and main bearings, connecting rods and rod bearings, camshaft and camshaft bearings, timing belt, chain or timing gears, intake and exhaust valves, valve springs, guides, oil pump, push rods, rocker arms, hydraulic lifters and rocker arm shafts. The Engine Block and Cylinder Heads are also covered if mechanical failure was caused by the above-listed parts. (Does not include any other than what is supplied in Long Block Assembly.) Seals and gaskets are not covered except when required in connection with the repair or replacement of the above-specified parts.

TRANSMISSION - All internally lubricated parts of manual or automatic transmissions, including: torque converter, oil pump, valve body, governor, bands, drums, planetaries, sun gear and shell, sprag(s), shaft(s), bearings and related bushings, shift rail, forks, synchronizers, transmission case, bell housing, oil pan(s) and all internally lubricated parts of the 4x4 Transfer Case. Damage resulting from failures by related parts or units such as but not limited to: levers, controls, linkage, radiator, coolers, rubber mounts, external oil lines and electronic components external to the transmission are not covered

  1. ELIGIBILITY:

To be eligible for the Product Warranty possibilities offered with this Customer Registration, the Major Drive Components (Engine and Transmission) and 4x4 Transfer Case (if applicable) must have been treated with Premium 2000 Plus Treatment Products and either

a)factory installed by the vehicle manufacturer

b)repaired or replaced previously only by an ASE Certified Mechanic (Documentation required)

c)replaced only by an ASE Certified Mechanic or the Registered Owner (as appears above) at Commencement Date prompting issuance of this Warranty.

  1. COVERAGES:

In case of failure of Premium 2000 Plus products to prevent breakdown within the lubricated parts of the engine and/or transmission of the covered vehicle, the Warrantor's obligation is limited to repairing or replacing defective parts and/or correcting defectively furnished labor including the replacement of all lost fluids (including Premium 2000 Plus products). The Limit of Liability per covered vehicle is $3,000.00 per Engine (aggregate) and $2,000.00 per Transmission (aggregate) including towing ($75.00 limit per occurrence) and Rental Car ($100.00 per occurrence). This Product Warranty is transferable to a second owner of the vehicle or to a replacement engine and/or transmission, provided the Authorized Administrator listed below is notified in writing and a $25.00 transfer fee is paid.

  1. EXCLUSIONS:

Failure of a covered component, which occurs within the first (90) days and 1,200 miles after Commencement Date (Today's Date), as appears on the above Registration Form, will not be covered. This product warranty does NOT apply to Diesel engines manufactured prior to 1990, Rotary engines, 7.3 Ford Diesel engines, 4100 GM engines, Renault Diesel and Gasoline engines, or engines used in commercial vehicles (call Authorized Administrator for specific clarification)

  1. EXCEPTIONS:

a)The Warrantor is responsible only for the engine and transmission purchased with the vehicle by the Registered Owner as appears on the above Registration Form, or any replacement component authorized by the Authorized Administrator.

b)The Warrantor is not responsible if the covered component(s) (engine or transmission) have been abused or damaged in an accident.

c)The Warrantor is not responsible if the covered component(s) have been tampered with outside a qualified repair shop after installation.

d)Abuse through towing, improper load capacity or continued operation of an impaired vehicle is not covered. If the vehicle has been used for towing purposes without the installation of a proper transmission cooler, any claim is not valid.

e)Blown head gaskets, oil leaks, cracked heads or block, overheating or other engine failure due to lack of fluids improper maintenance or pre-existing condition are not covered items.

f)Repairs because of collision, misuse, road conditions, negligence, alterations, racing, fires, floods, riots, vandalism, theft or acts of God are not covered.

g)This product warranty does not cover such things as loss of use of vehicle, loss of earnings, personal damages, per diem expenses or any other consequential or incidental damages.

  1. MAINTENANCE REQUIREMENTS:

To obtain the benefits provided under this Warranty it is the responsibility of the Registered Owner to:

a)assure that the covered component(s) are treated with the appropriate Premium 2000 Plus products within (60) days of warranty application and required payment.

b)assure that engine oil and filter are changed every (4) months or four thousand (4,000) miles, whichever occurs first. Receipts for oil and filter changes or oil and filter purchases (if changed outside a qualified oil change facility) will be required in the event of a claim.

c)assure that the transmission receives a tune-up at between 45,000 miles and 55,000 miles of this warranty. Included in this tune-up would be replacement of transmission fluid and filter and the addition of another treatment of Premium 2000 Plus Transmission Conditioner. This additional treatment will be mailed to the Registered Owner upon request as needed. Documentation of tune-up will be required in case of a claim.

d)continue maintenance of covered component(s) as outlined with the vehicles owner's manual. Service receipts and invoices may be requested in case of claim

  1. CLAIM PROCEDURE:

In case of a failure of a covered component, call the Authorized Administrator at 1-800-xxx-xxxx for claim validation. Be prepared to submit to the Authorized Administrator: a copy of this Product Warranty, original vehicle purchase invoice, any covered component purchase invoice, any covered component installation invoice, invoice documenting failure of any covered components, receipt for Towing and/or Rent-A- Car

If appropriate and proof of maintenance of covered components as outlined in Item 5 (Maintenance Requirements) of this Customer Registration. You will be required under this Warranty to authorize the repairing facility to tear down the component(s) for inspection before repair or replacement. You will be required to pay the cost of the tear down if the mechanical breakdown is not covered by this Warranty. Repairs or replacements must be completed by an ASE or ATSG Certified repair shop.

* Failure to receive the Authorized Administrator's authorization prior to any repair work will invalidate any

Warranty claim.

This agreement is a product warranty and is not insurance. It is not subject to state insurance laws but is subject to state laws concerning warranties. You must use the product as instructed in order to receive the benefit of the warranty.
