A. Hazardous Waste Satellite Accumulation Area (SAA)
1. Is the SAA identified by a yellow sign, located at or near the point of waste
generation and under control of the waste generator?
2. Are waste containers properly labeled with red and white URI hazardous
waste labels?
3. Complete chemical names are clearly marked on the label? No abbreviations
or chemical formulas?
4. Chemical concentrations are noted on label and add to 100%?
5. Contact information is included on label?
6. Are liquid hazardous waste containers stored in secondary containment
(grey bin)?
7. Are incompatible wastes stored in separate grey bins?
8. Are waste containers compatible with waste they are holding?
9. Are waste containers in good condition?
10. Are waste containers securely closed? (Is the proper cap/lid used, no funnel
left in opening?)
11. Are any containers full and ready to be picked up?
12. Has a Pick-up Request been sent to EHS?
13. Have obsolete or unused chemicals been properly disposed or donated to the
chemical reuse program?
B. Personal Protective Equipment
1. Eye protection and gloves available and worn appropriately?

C. General Work Environment
1. Complete chemical names are used on container labels (No abbreviations or formulas)
2. There is unobstructed egress to emergency exits throughout the lab?
3. The areas around fire extinguishers, pull alarms, emergency showers, eyewash stations
and electrical panels is kept clear?
4. Gas cylinders are properly secured?
5. Bench pads are clean and benches free of clutter?
6. Broken glass disposed in a cardboard box; not overfilled?
7. Food/drinks not stored/consumed in lab area/refrigerators?
8. Floor clear of slip/trip hazards; no bottles on floor?
9. Flammables are stored in flammables cabinet when not in use?
10. Extension cords and power strips are not daisy-chained together and no
permanent extension cord is in use?
11. Ventilation good; no odor present? Hood certification is current?
