We want to thank you for your interest in participating in the Environmental Modification (EM) program to assist our consumers to continue to live in their homes.

There are several aspects of the Environmental Modifications (EM) program that potential contractors should understand prior to contracting with Seniorsplus/Waiver Services Provider (WSP).

·  First, due to the billing procedure with the state of Maine, contractors will most likely not be paid for the work completed for three to six weeks after your invoice is received by WSP.

·  The work completed must be by ADA/State/Local standards and ramps must be built at most at 1:12 pitch and no steeper. Ramps and outdoor work needs to be completed with pressure treated wood.

·  It is WSPs expectation that your work would be inspected by a second party and WSP would receive documentation of that inspection.

·  Detailed before and after photos of work area must be submitted along with the final invoice.

·  Please understand that no monies can be fronted for materials or labor as the environmental modification work needs to be completed before submission of the billing to the state and subsequent payment to the contractor.

·  If state funds are used for an EM that is found to be built out of compliance with ADA/State/local standards or found to be fraudulent in any way, those funds will need to be paid back to the funding source by the contractor.

In order to eliminate confusion the following items will be required for the Contractor Agreement:

·  All proposed estimates will have itemized pricing.

·  The items will include but are not limited to the following; the size and length of the landing/ramp the total number of hours, cost of labor hours, the cost of all individual materials and any other supplies applicable to the job.

·  Any work that is to be completed by the consumer/family will be listed on the estimate.

·  The contract is between SeniorsPlus and the contractor and no modification of the contract shall be made without express written consent of the WSP Coordinator.

Once WSP receives your quote for an EM job/request, please understand that this amount is what the final payment will be. The EM program does have financial limitations that we cannot exceed so the quote submitted and accepted is the final amount that will be paid.

If you wish to proceed with becoming a SeniorsPlus/WSP EM contractor please complete the application enclosed and return it in the self address envelope enclosed. Please also sign this letter indicating you have read and understand the parameters of this program as described above.
