Job Title:SAfER Project Lead(Age UK South Staffordshire)

Location:Based at Roller Mill but working in South Staffordshire District

Salary:£22951.00 pa

Hours:37.5 Hrs per weekfor 12 months from starting date.

Job Purpose:

To take the lead role of an exciting new project inSouth Staffordshire locality 3,4 and 5 working in partnership with the practice teams, to provide identified patients with volunteer led support and sign-posting service, coordinating the support offered within the community from the voluntary and statutory sectors and ensuring they experience ‘joined-up’ services. This service will be adapted to meet the needs of the individual needs

To lead working in partnership with Staffordshire Fire and Rescue to deliver to older people a volunteer led support and sign-posting service, coordinating the support offered within the community from the voluntary and statutory sectors and ensuring they experience ‘joined-up’ services which meets individual need. This service to be delivered in South Staffordshire and in Tamworth in nominated districts.

Key Duties and Responsibilities

  1. To work with the practice staff to identify patients who would benefit from access to appropriate services in the community and support from volunteers; such patients may be socially isolated with limited family or care networks and may be at risk of deterioration, loss of independence and admission to hospital or care homes.
  2. To develop a referral process which links to the Staffordshire Fire and Rescue Service (SFRS) evaluation process.
  3. To train the SFRS staff on the referral process and evaluation criteria which link to the defined responsibilities of Age UK.
  4. To organise the recruitment and training of all volunteers.
  5. To develop and maintain a detailed knowledge of local community and voluntary sector services, including access arrangements and eligibility criteria.
  1. Toprovide identified patients with a time-limited service, linking statutory, community or voluntary sector providersand volunteer support. This will include wider services or activities that may help promote independence, health & wellbeing, mobility and community cohesion.
  2. Where necessary the Service Lead may accompany patients on their first visit to a service or arrange for an Age UK volunteer to do so.
  3. To facilitate coordination of input from different professionals where patients have complex needs or may be at risk of falling through gaps between services. (NB – this is not a ‘case management’ role – the supporting task is to provide a time-limited intervention to identify what the patient needs and support them in obtaining it.)
  4. To develop relationships and linkages between volunteers and community & voluntary organisations.
  5. To provide an ambassadorial function for the partners in relationships with the voluntary sector, volunteer networks and other community groups.
  6. To support the development of networks for referred and nominated individuals (with Long Term Conditions) within the community and enable them to access advice and education from experts and link with existing networks.
  7. To keep records for project monitoring/evaluation requirements, feeding back on service quality and accessibility as required and identifying opportunities/gaps in service. This may include the production of ‘case studies’ to illustrate the service provided.
  8. To manage own workload through planning and organising the work schedule, obtaining and organising the necessary information and resources.
  9. The post holder will be required to have an awareness of the relevant Age UK policy and to work within promulgated provisions.
  10. This post is developmental and the post-holder will be expected to work with managers and colleagues to amend the job description and main duties in light of experience. This could include other duties not foreseen in accordance with the general job purpose and grading of the post.

Terms & Conditions of Service

  1. The post is based at the Roller Mill, and will engage with relevant GP practices and referred individuals in nominated areas.
  1. Provisions relating to working conditions and sickness allowance are covered by existing terms and conditions.
  1. The post is temporary to the 31st March 2016 in the first instance. Hours of work and working pattern negotiable. Duties may involve very occasional evening and weekend work for which time off in lieu will be granted.

4Annual leave entitlement will be 25 days per annum (pro rota), 30 days for those officers with five years service. Bank holidays also apply. (Pro-rata for staff working part-time).

5The appointment is subject to one month's notice on either side.

6The postholder will be required to meet the travel requirements of the post; if using a car an agreed mileage rate will be paid. It is a condition of employment that the postholder remains eligible at all times to drive such a vehicle.

7Pension Scheme details will be issued with any formal offer of appointment.
