Basic Scholarship Essay

Please respond to one of the following questions in a short essay. The essay should address one of the questions and be a maximum of two pages in length, double spaced in 12 point font. Please be aware that the essay must accompany the scholarship form.

1. Describe a past life changing event and how it made you who you are today.

2. In twenty years, where do you see yourself and how will you contribute to society.

Scholarship Requirements

Recipient must be qualified in one of the following three categories:

1. Mother was a HEAL recipient

2. Mother currently undergoing cancer treatment or went through cancer treatment in the last year

3. Mother has passed away from cancer

* Recipient should be a high school senior through junior year in college

* Recipient Selection will be based on GPA, extra-curricular activities, essay and financial need

*Dates for entry- Please find the application on the HEAL website at: www.happilyeverafterleague.org/scholarshipfund

Applications are due by April 10, 2011, no exceptions. Winner will be notified no later than April 25, 2011.

*Note-Previous scholarship winners are not eligible to apply for the 2011-12 Laura Mylonas Scholarship due to the large number of applicants.

*Presentation of Award- The scholarship(s) will be presented with a certificate at the Fairytale Tea on May 7, 2011 at the Scottsdale Plaza Resort. Please note the actual scholarship check will be made out to the college of the recipient's choice and given to the recipient when their tuition is due (or in advance but the check will be made out to the college or university and not the student.)
