HealthCare Volunteer - Kool Smiles Pro Bono (Free) Dental Care Program.
Requirements:
1) Patient cannot have current dental insurance (private or public insurance).
2) (Adjustable Gross Income - AGI) Income for previous tax year must be under Federal Poverty Level (FPL). Proof of AGI must be provided.
3) Copy of last year's tax return (last available) or Statement that no tax return was filed.
4) After acceptance of your registration patient will be given health history form to fill out in person. After turning that in to staff members, patient can schedule a patient at 8:30am on select Saturdays. There are 2 openings per Saturday.
Details: Patient will pay for any dental laboratory costs and understands that dental care on patient can be discontinued at the discretion of HealthCare Volunteer and Kool Smiles at any time due to funding or personnel limitations. All ages are eligible and care is provided on a first come first serve basis. Palliative and emergency patients are given priority over waitlist patients. Due to the large waitlist there is no guarantee patient will receive free dental care or continued dental care.
Times: Appointments are typically available Saturday morning at 8:30 am, and some mornings/evenings on weekdays on a very limited basis.
Treatments Available: Crowns, Fillings, Extractions, Minor surgeries, Palliative treatment, Obstructive Sleep Apnea dental devices (snoring devices), Perio Protect, TMD. No endodontia or cleanings/prophylaxis.
