EMERGENCY PROCEDURES:

SC end everyone goes into the hallway and closes their doors AC3, AC1 and AC2 all meet in AC3

PARKING VEHICLES:
Upon arrival turn vehicle off and leaves keys in the ignition
GH, Roc Vale staff turn keys in to the office
When returning from outings park vehicles in the vehicle parking spaces and turn keys in to the office.
Never park a vehicle in front of door 1. This is reserved for emergency purposes.
BREAKS PROCEDURE:
Sign out/in sheet located at the front desk
Only one staff from the same classroom goes on break at a time
Two 15min. Breaks per staff in one day. (One in the morning, one in the afternoon) / Date / Staff initials / Trainers Initials
Break Areas (Break Room, Smoking Area on side of building only)
PERSONAL BELONGINGS/CELL PHONES:
Personal belongings are not allowed in the classrooms (No food, purses, coats etc.)
Get a lock and key from the Secretary to lock up personal belongings
Cell Phones (need to be turned off when in the classroom and are only to be used on breaks and in break areas)
CLASSROOM ASSIGNMENTS/PROGRAMS:
Census Books (explain the content and always take with the group during any transition times including outings, emergency drills, outside, living skills)
Active Treatment
Documentation (All programs need to be documented when completed)
Read Program Cover Sheets and BDP=s while in the classroom
Reinforcers (should be picked up at front desk if there are none in the classroom)
REPORTING INCIDENTS, INJURIES AND BEHAVIORS FOR INDIVIDUALS:
All need to be reported to the SOD immediately
All need to be documented on the back of your census sheet
Behaviors need to be documented on ABC sheets and turned in to the office immediately or documented on data sheet if applicable
Incidents (no visible injury) a body check completed immediately by the staff reporting the incident
Injuries- an injury reports needs to be filled out immediately and turned in to the SOD
REPORTING STAFF INJURIES:
Staff injuries need to be reported the SOD immediately and a staff injury report filled out and turned in
LUNCH TIME/ LIVING SKILLS PROCEDURES: / Date: / Staff initials / Trainers Initials
One staff and one individual from each classroom should set up the classroom lunches in the gym
Each classroom has an assigned lunch table
All dirty dishes need to be taken to the kitchen after lunch
All food and beverages need to be opened and offered to each individual
All individuals who ambulate need to be taken to the bathroom to be cleaned up after eating and before coming up to the front desk for money management programs
Review adaptive equipment location
All spills during meals need to be cleaned up immediately
When making snacks/meals in living skills all dishes need to be washed, tables and counters clean, any left over food thrown away.
CONTINENCE PROCEDURES/PROGRAMS
When individuals are in the changing rooms close the privacy curtains and in the bathrooms close the door
Never leave an individual unattended in the bathroom/changing room
If you need undergarments, clothes for an individual, press the call button and asked for the SOD to bring them to you
Always put a chux down on the changing bed/table before repositioning the individual on it.
Lock changing table rails in place once the individuals are on them
Wipe down changing bed/table when finished changing the individual. Use Sani cloths located in the changing room
All dirty clothes should be put in plastic bags and placed in the dirty laundry barrels not on the floor of the hallway or changing room
All urine accidents need to be cleaned up off the floor, toilet immediately.
TRANSPORT/ ARRIVAL/DEPARTURE/VEHICLES / Date / Staff initials / Trainers initials
Upon arrival to MTC assist individuals with getting on and off of vehicles
Transport (review transport sheets, assignments)
Always walk individuals to their assigned classrooms and stay in the room with them until you get coverage
Fueling Vehicles (where to go, how to do, fill if less than 1/4 tank)
Lift van (how to strap, working controls, lift)
When you arrive to individuals homes always walk them in
and make sure there is a staff on their home before leaving
OUTING PROCEDURE
Outings are posted with the daily assignment sheet
Staff need to get outing materials (money, cell phone, back pack from the Secretary)
Sign out for outings at the front desk before you leave
Only purchase items equivalent to what the individuals are purchasing
All change, receipts, keys, cell phones and back packs, need to be turned in to the Secretary when the group returns
One staff needs to fill out an outing report and turn it in to the office immediately
WORKING WITH NON-AMBULATORY INDIVIDUALS:
How to use an easy lift and slings
Location of extra slings
Seat belts (always buckled unless BDP says otherwise)
Shoulder straps and ankle straps (when to wear, info in IHP)
Gait Belts/Walking Belts (how to use appropriately)
Pivot Transfers (how to do)
Handicap Bars (locations and how to use)
