ID Badge and Access Card / ·  Complete email template provided on our onboarding system, take a professional photo, and submit both to
·  Once your ID card is ready you will be contacted for pick-up or delivery
Communicable Disease Assessment (CDA) / ·  Visit ‘Employee Immunizations New Employee Page’ to find the CDA process and access the CDA form
·  Submit your completed CDA form to or fax (780) 670-3622
·  Once contacted by AHS Workplace Health and Safety (WHS), attend a CDA clinic appointment with AHS or Public Health
Orientation – Part One / ·  Complete Part One – Orientation
·  You will receive three hours pay for completing the Orientation.
·  Print the certificate of completion at the end of the orientation and provide it to your manager on your first day of work
Benefits and Pension / ·  Review benefits and pension options
AHS Code of Conduct / ·  Familiarize yourself with the AHS Code of Conduct
Parking / ·  Fill out theParking Applicationform and send it to your local parking office. Include the name of your work site in the subject line of your email.
Note: Parking availability is not guaranteed.
License and Professional Membership / ·  Requirement for: Regulated health professions under theAlberta Health Professions Act and Health Disciplines Act. Other professions governed by government acts and collegeor regulatory body requirements may be required to provide additional documentation.
·  Upload your license/practice permit using our onboarding system.
Criminal record check and/or Vulnerable Sector Search / ·  Apply at your locallaw enforcement officefor a criminal record check and/or Vulnerable Sector Search.
·  Upload Criminal Record Check using our onboarding system.
Basic Life Support - Health Care Provider Level (BLS-HCP) Certification / ·  Upload your Basic Life Support (BLS) Certification using our onboarding system.
Education Allowance / ·  Employee groups: AUPE – Auxiliary Nursing, HSAA – Technical, and UNA
·  Your collective agreement provides more details
·  Complete the Education Allowance Form
·  Submit your form using our onboarding system

Set up Direct Deposit Information / ·  Enter your personal banking information into the AHS e-People system within five days of your start date.
·  The e-People User Guideexplains how to add or edit your direct deposit banking information.
Canada Revenue/ Personal Tax Credit Returns / ·  TD1 2017 Personal Tax Credit Return - Federal
·  TD1 2017 Personal Tax Credit Return - Provincial
·  Send completed and signed forms by fax at 1-888-908-7265 or to
AHSecure – Collect It Protect It Privacy and IT Security Training / ·  This mandatory course is available onMyLearningLink, and takes about 20 minutes to complete.
·  You will need to electronically sign the Confidentiality and User Agreement at the end of the course

Benefits and Pension / ·  Complete enrollment in AHS benefits and pension plans
Recognition of Previous Experience / ·  Employee groups: AUPE – Auxiliary Nursing, HSAA, and UNA
·  Fill out theRecognition of Previous Experienceform ‘SECTION I’, print and sign.
·  Send the form to your previous employer(s). Please note: for ‘SECTION II’, a separate form is required for each employer.
·  Once you have received all your completed forms back from your previous employer(s), send this information by fax to your local HR office as indicated on theform.
Long Service Pay Adjustment / ·  Employee group: UNA Requirement: 20+ years of nursing service
·  Once you have your completed form back from your manager, send this information by fax or email as indicated on the form.
Orientation – Part Two / ·  This module provides additional information and resources to guide you through your first three months of employment with AHS
·  Register for the Orientation Passport through MyLearningLink
Union Orientation / ·  If you are a member of a union, you can attend a union orientation to review the terms and conditions of yourCollective Agreement
·  Theunion presentation calendarprovides dates, locations and schedule of future sessions. If you are scheduled to work, you will need to make arrangements with your manager to attend
