Notes:

1.  Only enter the start and end times of the overtime hours worked excluding the ordinary time worked.

2.  A minimum 0.5 hour (30 min) meal break should be taken as necessary to ensure that the staff member does not exceed five continuous ordinary hours of duty without a break.

3.  Signatures of staff member and supervisor confirm the details provided at Columns A-H. For a staff member in receipt of a salary not exceeding that prescribed for the top of the scale for HEW level 7, the signatures also confirm agreement to TOIL prior to the authorised overtime being worked.

4.  Signatures of staff member and supervisor confim the date that the accrued TOIL is taken.

5.  TOIL Record required to be maintained in the staff member’s workplace and retained for a minimum of eight years from the date of TOIL accrual in accordance with the Monash University Retention and Disposal Authority.
