I will  / will not  be able to provide transportation for the trip. I can accommodate ______passengers in the car besides myself. If your car has an air bag, the Maryland State Police recommends that children in third grade and younger ride in the back seat only.

For children 8 years old and younger: Maryland law requires children to be in a car or booster seat until the age of 8, unless they weigh more than 65 pounds or are 4’9” or taller. If your child requires a car or booster seat, you must provide one in order for your child to attend the trip. If the school is not provided with a car or booster seat, your child will not be able to attend the trip.

My child will  / will not need a car or booster seat. If one is necessary, I understand that I will provide one for the trip or my child will not be able to attend.
