6) BRIEF RESUME OF THE INTENDED STUDY:

6.1) NEED FOR THE STUDY:

The advancement of busy, professional, sedentary, social life, every human being is under pressure. Faulty life style is responsible for changes in the bodily tissue and plays a vital role in the manifestation of many diseases. In this way progressive disorders affecting the joints of human body is now becoming a significant treat to the working population.

Apabahuka is a diseases which affect the shoulder joint and nearer structure. In Ayurvedic classics Apabahuka is a diseases due to vata and kapha involvement. It affects the Amsa Sandhi (shoulder joint).In Apabahuka shoola during the movement and stabdata are the classical signs.1 It is a diseases which hamper the day to day activity of an individual

Panchakarma is a very unique therapeutic procedure because of its preventive promotive,prophylactic and rejuvinating properties as well as providing a radical cure. Among the panchakarma the nasya karma is a chikitsa that is applicable in urdwajatrugatha vyadhis2.

In classics different therapies like Nasya,Snehapana,Abyanga,Swedana and Shamana chikitsa is mentioned for Apabahuka. 3In contemporary medical science potent Analgesics,antiinflamatory drugs and also Corticosteroids are prescribed for the diseases.4But these drugs are not so effective and pose increased risk of gastric erosion, hepatic and nephrotoxicity etc adverse effects.

In view of this we have designed a clinical study to evaluate the efficacy of Nagara taila through nasya in the management of Apabahuka.

6.2) REVIEW OF LITERATUR:

·  Nidana and Samprapthi similar to Samanya vata vyadhi5.

·  Lakshanas,and Chikitsa of Apabahuka mentioned by Acharyas6.

·  Nasya is mentioned detailed by Acharya Vaghbhata7.

·  Many taila yogas are explained for the treatment of Apabahuka but here an attempt is made to evaluate the efficacy of taila processed with single drug Nagara

·  Criteria For Selecting Nagara.

1.  It is one among the drugs told in”Vatagana Gana” in Vata Vyadhi Chikitsa Adhyaya of different classics.8

2.  Nagara is kapha Vata shamaka in action. Apabahuka is due to involvement of Vata and Kapha.9.

3.  Nagara is having shoola hara property mentioned in “Shoola Hara dashemani”.10

4.  In all most 70% of yogas told in Vata Vyadhi Chikitsa in classics Nagara is present.

5.  Nagara have synonyms like Maha-aushada,Viswabeshaja,so it may have multiple action.

6.  Widely available, cheaper and non controversial drug.

7.  Nagara is told as a substitute for Rasna.(Beshaja kalpa by Baradwaja)11,and Rasna is considered as the best medicine for Vatika disorders.12

·  Criteria For Selecting Taila

  1. Taila is having the property to reduce vata and it is not increasing Kapha. Taila is increasing bala.13
  2. In Vatavikaras Brimhana is to be adopted and taila has this property.14
  3. In vata Kapha vikaras taila is indicated .15
  4. To reduce the intensity of rukshata in Nagara,Taila is used.
  5. Taila is having mrudhukara bhavas.In Apabahuka akunchana (constriction) is there. This akunchana is relieved by mrudhukara bhavas of taila.

·  Criteria A For Selecting Nasya

1.  In Skanda gatha Vata treatment, Nasya is mentioned.16

2.  Apabahuka can be relived by therapies like Nasya and Sweda according to the classics3.

3.  According to classics, the drug administered through the nose enters in the Uttamanga and eliminates the morbid doshas residing there.17

·  Review of such conditions which are also explained in contemporary medical science will be undertaken. Relevant journals, periodicals, magazines, etc. Books and research conducted at different centers will also be reviewed.

·  Previous Work Done:

Many studies have been done in many P.G.Centers all over India under various universities.some of these are-

1.  GEORGE P.J. - Classical management of Apabahuka by Kerala University, Thiruvananthapuram(1981).

2.  PHILOMINA P.J. - Apabahuka and its management by Government Ayurveda Medical College, Mysore (1988).

3.  SHUKLA NIRANJAN - Effect of Nasya and Abhyanga on Apabahuka w.s.r to Masha Taila by Gopabandhu Ayurveda Mahavidyalaya, Puri(1997).

4.  NAVEEN C.J. - A study on Apabahuka and its management through Nasya Karma and Vatagajankusha Rasa. By Dr B.N.M Rural Ayurvedic Medical College, Bijapur (2004).

6.3) OBJECTIVES OF THE STUDY:

1.  To evaluate the efficacy of Nagara Taila through Nasya in the management of Apabahuka.

2.  To study Nasya in detail.

7. MATERIALS AND METHODS:

7.1 SOURCE OF DATA:

a)  Subjects:

Patients suffering from lakshanas (signs & symptoms) of Apabahuka will be selected from O.P.D and I.P.D, departments and also from mobile camps of K.V.G. Ayurveda Medical College and Hospital, Sullia.

b)  Literary:

Literary aspect of study will be collected from Ayurvedic classics and Modern text books & will be updated with recent medical journals and internet.

c)  Selection of drugs:

The trial drugs will be identified and collected from surrounding areas (locally).

7.2  METHODS OF COLLECTION OF DATA :

(a) Study Design:

A comprehensive clinical study.

(b) Inclusion Criteria:

1.  Patients above 20 and below 70 yrs of age.

2.  Patients of clinical features of Apabahuka.

3.  Patients of both genders who are Nasya yogyaa18 according to Ayurveda classics.

(c) Exclusion Criteria:

1.  Patients below 20 and above 70 yrs of age.

2.  Patients who are Nasya ayogyaa19 according to Ayurveda classics.

3.  Patients with the history of fracture and dislocation of affected hand.

(d) Diagnostic Criteria:

Diagnosis will be established by clinical examination (signs and symptoms) of Apabahuka as per our classics.

(e) Therapy: Nasya with Nagara Thaila

Minimum number of 30 patients fulfilling the inclusion criteria will be randomly selected irrespective of sex.

Procedure of administration of treatment:

30 patients will be treated with Nasya, given with Nagara Taila for 7 days with appropriate dosage as per classics.

(f) Assessment criteria:

Subsidation of signs and symptoms of the disease and attainment of healthy signs (Arogya Lingani).

(g) Preparation of medicine:

Ingredients of Nagara Taila:

1.  Kalka & Swarasa of Nagara.

2.  Tila Taila.

Taila will be prepared according to procedure of Taila Paaka Vidhi mentioned by Ayurvedic classics.(Sharangathara Samhiha) .20

(h)Posology: dosage will be fixed to 6 Bindus.

(i) Study duration:

·  Nasya karma: 7 days.(minimum)

·  Follow up: 14 days.

·  Total duration: 21 days.

7.3. DOES THE STUDY REQUIRE ANY INVESTIGATION OR INTERVENTION TO BE CONDUCTED ON PATIENT OR OTHER HUMANS OR ANIMALS? IF SO PLEASE DESCRIBE BRIEFLY.

The study will be conducted clinically. No animal experimentation will be carried out.

Investigations: For diagnostic purpose (if necessary)

·  X-Ray of shoulder A/P & LAT .

·  Routine blood & urine examination.

7.4 HAS THE ETHICAL CLEARANCE BEEN OBTAINED FROM YOUR INSTITUTION?

Yes, the clearance has been obtained through the ethical committee formulated in the institution.
