FINAL REPORTGUIDELINES AND FORM
As indicated in your grant award letter, The Chicago Community Trust requires your organization to submit a Final Report including narrative and financial information on activities related to the grant. Your submission of theFinalReport is important to the Trust as it is an integral part of our monitoring and evaluation process of your individual grant, or a set of grants, made to support a key strategy or program initiative.
- Always download the form from our website to ensure you are submitting the most current version of the Final Report form.
- The Final Report includes a Narrative and a Financial Report, both of which are due on or before the final report due date indicated in your grant record form. Please note that failure to submit the Final Report by the due date will affect your pending, or future, grant application with the Trust.
- NarrativeReport: Answer all questions directly on the report form. The final narrative report should present a review covering the entire grant period. The successes of your project and the challenges you faced are important to capture in the narrative, since they serve as a valuable learning opportunity for Trust staff and your organization. To this end, the Trust appreciates your candor and thoughtful review of your project experience.
- FinancialReport: Complete all information requested on the attached form. If there were significant changes to the Project Budget from what was originally proposed, please explain. If there is a positive balance from the grant, please explain the reason. If the organization wishes to use the balance for a purpose related to the original project, please submit a written request to your Program Officer, who will need to approve the use of the balance.
- Submission of the Final Report is an indication that your project is complete and that all grant funds have been spent. If you anticipate that the project can not be completed by the time the Final Report is due, you must contact your Program Officer prior to the due date to request, in writing, a no-cost extension.
- If the project was evaluated by an external evaluator, in addition to the attached reports, please include a full report outlining the main questions framing the evaluation of the project and measures used to answer those questions. Include the evaluation findings for each question. Also include references to the data collected and how it was analyzed. Describe any adjustments to the evaluation approach that were made during the evaluation.
