CALL TO ORDER
EXECUTIVE SESSION
The motion was made and passed to go into Executive Session pursuant to Virginia Code, 2.2-3711-A (15), for the purpose of conducting a Next Friend review. Members present were Michael Williams and Shannon Slaughter. Also present were Horizon Behavioral Health representative Matilda Scott, as well as providers Bethel and Carleton Thomas, Gail Ballowe and the consumer.
The motion was made and unanimously carried to appoint Gail Ballowe as Next Friend for this individual.
The motion was made and passed to go into Executive Session pursuant to Virginia Code, 2.2-3711 (15), for the purpose of discussing the restrictive treatment plans for individuals receiving service. This review necessarily involved discussion and consideration of mental and medical records excluded and information excluded from the Freedom of Information Act. Members present were Michael Williams and Shannon Slaughter. Also present were DePaul Community Resources’ representative Ryan Long and guardian Ruth Givens.
The motion was made and unanimously carried to approve the plan, to be reviewed in one year.
Four other plans were presented by Ms. Long. One was tabled until November, pending additional information, 2 were acknowledged and do not have to be heard again, and the fourth is pending additional information in the plan [Note: this was submitted on 9/26/16].
NEW REGIONS – Ms. Mayhew distributed copies of the new regions map and will e-mail a color copy to all. Note that Lynchburg and Danville are no longer in the same region.
APPROVAL OF DRAFT MINUTES - The Committee unanimously passed a motion to approve the draft minutes from the July 12, 2015 meeting, with an addendum.
HUMAN RIGHTS ADVOCATE’S REPORT
New Contact - Ms. Crowder announced that she was filling in for Tammy Long, who began as advocate for Region 4 this month. Our contact person is currently Cassie Purtlebaugh, at
804-382-3889. Interviews are now being conducted for our vacant advocate’s position.
Revised Regulations – are now on the secretary’s desk at the governor’s office, but there is no specific date for signing. These will hopefully be implemented by January 2017, after which training from the State will begin for all. Some significant changes may be forthcoming.
Investigations Committee – statisticians will be randomly visiting providers to study the process in the State; one important aspect they will be examining is how well investigations are documented. Ms. Purtlebaugh will accompany them.
Change re: Affiliation Requests – Ms. Crowder said that affiliation requests will be automatically granted, meaning that the Committee does not need to hold a vote. Attendance at the meeting is still necessary. Ms. Mayhew will not be providing letters, unless specifically asked to, because the minutes will reflect the information.
PUBLIC COMMENT TIME – Mr. Pyzik from Anderson Counseling said he is having a terrible time with Delta regarding the CHRIS system. Others are as well. One resource for getting credentials for the CHRIS system is Keanna Bobbitt at DBHDS.

“SAFETY-CARE BEHAVIORAL MANAGEMENT SYSTEM” – Lutheran Family Services
asked for approval of this new program.
The motion was made and unanimously carried to accept Lutheran Family Services’ change to the Safety Care behavior management system and their policies reflecting this change. A letter confirming this approval will be sent by Ms. Mayhew on behalf of this LHRC.
PROGRAM UPDATES
Anderson – serves 140; 6 complaints of neglect due to peer-to-peers. The Arc of Central Virginia – 73 individuals, 3 sponsored homes, 3 in-home, no complaints or allegations of abuse/neglect. Bedford Adult – is licensed for 50 and is approaching capacity, no complaints or allegations of abuse/neglect. Blue Ridge Behavioral Healthcare – no complaints or allegations of abuse/neglect. Blue Ridge Residential –no complaints or allegations of abuse/neglect. Braley & Thompson – serving none at this time, no complaints or allegations of abuse/neglect. Centra – 250 patients per month; 1 abuse and 1 complaint allegation. Creative – 65 individuals; no complaints or allegations of abuse/neglect. DePaul –3 neglect allegations. Dominion – serves 89, no complaints or allegations of abuse/neglect. Foundation Stones – serves 12; no complaints or allegations of abuse/neglect. Genesis Manor – 6 individuals in 2 homes; 1 peer-to-peer, neglect unfounded. Harmony Day Support – serves 33; entered 1 on CHRIS. Harvest Outreach – serves 71, had 4 peer-to-peer. Heart Havens – 4 clients in house 1, house 2 is still empty, no complaints or allegations of abuse/neglect. Horizon Behavioral Health – serves 9016; 3 complaints, 6 abuse/neglect allegations, 1 physical abuse complaint pending. HumanKind –
no complaints or allegations of abuse/neglect. Immanuel – was licensed 9/16 and is in the process of finding homes and individuals. Landmark Haven – serves 3, no complaints or allegations of abuse/neglect.
