Service delivery
We’ll complete assessments for all our service users at a location that best suits their needs. This can be at one of our friendly, accessible hub and local delivery point (LDP) venues, in a convenient GP surgery, or in an appropriate community location.
Our hubs will be located in Hinckley and Loughborough with LDPs in Coalville, Melton Mowbray, Market Harborough and Wigston.
Employing a talented team of workers
The Leicestershire and Rutland drug and alcohol service will be made up of three teams, each managed by an Operations Manager and supported by a Senior Practitioner. We’ll have Substance Misuse Workers who are multi-skilled, appropriately trained and competent to work with both drug and alcohol misuse service users. Supporting the service we will also be providing a doctor, CPNs and nurse prescriber.
We’ll also use our Recovery Workers to support service users to put in place steps that will help them move through treatment, achieve planned exits and reintegrate into the community. They’ll take the lead on ensuring the ‘wraparound’ needs of our service users (both during and after treatment) are met.
Service users
We’ll make sure that we involve service users in every stage of the new service.
We know that it’s important for service users to receive clear information about what the new service will look like. This information will help to bring the service users with us and, when change happens, the information we provide will help service users be a part of the changes.
Support for families and carers
At Swanswell, we see families, carers and concerned others as being keyto affecting change in a service user’s life.
In our experience, involving them in the recovery journey can help to increase the likelihood of the service user achieving their goals and maintaining a more positive lifestyle. We will provide access and support to carers and friends, either as part of the service user’s care or independently of service users.
Partnership working
We know how important it is to build effective working relationships with other agencies that can support our service users to achieve their overall recovery goals and reintegrate into the community.
We’ll work with the DAAT and local authorities to identify key stakeholders that will be instrumental in developing relevant pathways and to cement excellent working partnerships.
