Service Coordinator

General Description:

To oversee the coordination and scheduling of direct services provided to all individuals we serve.

Competencies/Guiding Principles:

·  Schedule Direct Support Staff based on the needs and desires of those we serve.

·  Understand philosophy of self-determination through the person-centered planning process for those associated with the local mental health agency

·  Ability to establish a good rapport with a variety of people, both with and without disabilities.

·  Manage and monitor individual mental health budgets/DHHS awards as they relate to scheduling

·  Ability to be nonjudgmental

·  Meet face-to-face with individuals we serve a minimum of once every 45 days to determine level of satisfaction with Direct Support Staff and agency in general (satisfaction surveys can be used for this)

·  Act as liaison between those we serve, the local mental health agency, and other community agencies to build and maintain professional relationships

·  To learn and use active listening skills

·  Evaluate and assess needs of individual to best match with Direct Support Staff

·  Be conscious of my own values and how they influence my professional decisions

·  Provide advocacy when the individual’s preferences, needs, or talents are neglected or overlooked

·  Maintain availability to respond to situations beyond the capabilities of the Direct Support Staff, including performing duties as Direct Support Staff when coverage is unavailable.

·  Must be able to maintain strict confidentiality to any sensitive/clinical information

·  Attend team/staff meetings as needed

·  Be part of an on-call rotation

Education and Experience:

Required: High School Diplomas or GED. One (1) year professional or personal experience working with people with different disabilities required. Knowledge of community agencies helpful but not required. Valid driver’s license, reliable vehicle, current insurance and registration. Some travel

within the county required. Must be able to pass a back ground check & drug test. Knowledge of basic computer skills with the ability to learn new systems on an as-needed basis.

Abilities: Ability to work independently with minimal supervision as well as work in a team setting, meet deadlines, adapt quickly to change, demonstrate assertiveness and initiative; time management, and record-keeping skills. Effective communication skills both orally and written. Ability to identify and resolve conflicts that may arise with employees and/or those we serve. Organizational skills are essential as well as being able to prioritize tasks each day.

Work Environment: Work is performed mainly in an office setting and occasionally in an individual’s home. This position is meant to be flexible with the hours, although most work is done Monday – Friday, 9a – 5p.
