Disc Jockey Questionnaire
Please complete the following questions to the best of your ability and return to The Great Hall no later than 10 days before the date of your reception. Once your questionnaire has been received you will be contacted by the disc jockey. If you have any questions please call The Great Hall at (412) 366-2610 and leave a message for the disc jockey. Your call will be returned in a timely fashion. Thank you!
Directions:
§ Use the “Tab” button to move between fields within each table
§ Pressing the “Tab” button on the last row of a table will add a new row to the table. If you do not need the new row you can go to “Edit” menu on the toolbar and select “Undo”. This will remove the row. Or, you can highlight the entire row and go to the “Table” menu from the toolbar and select “Delete à Rows”.
§ Use the mouse or arrow keys to move between tables.
§ Please replace all of the information contained within the brackets, “[]”, with your own information.
§ Please remove italics from your final answers. [Hint: When you have completed the entire questionnaire press “Ctrl + A” to select all and then press “Ctrl + I” to remove italics.]
Currently, we cannot accept questionnaires via email. We will be adding this functionality in the near future. Please mail the completed questionnaire to:
