Position Summary:
As part of a team, the Resource Navigation Specialist will provide reliable and objective information about a broad range of programs and services for older adults, people with disabilities, caregivers, and veterans through helping individuals evaluate various options through assessment and application assistance. The Resource Navigation Specialist additionally provides reviews to provider database.
The following responsibilities are representative of recurring tasks. These are not to be construed as exclusive or inclusive. Other duties may be required and assigned. These responsibilities must be performed satisfactorily with or without accommodation.
Position Responsibilities:
· Screen, triage calls and visitors from the general public regarding services available for older adults and individuals with disabilities, their families and caregivers in a polite and professional manner.
· Provides person-directed decision support on options of a full range of private and publicly funded long-term care and service options available to caregivers based upon a thorough review of their personal history, preferred lifestyle and goals for the future.
· Complete prescribed assessment tools as requested.
· Uses resource and client tracking databases, including how to search for services, retrieve information, and document consumer contacts and follow up.
· Ensures accurate information database through maintaining a comprehensive review and update of the provider database.
Essential Skills, Knowledge and Abilities:
· Strong interpersonal skills to relate to diverse cultures, elderly and persons with a disability and their caregivers.
· Excellent verbal communications skills
· Knowledge of programs and services available for the elderly and persons with a disability.
· Client assessment skills with ability to determine needs through fact finding and make decisions through independent judgment of options.
· Recognize when a caller is experiencing a crisis and provide appropriate assistance and referral.
· Attention to detail in maintaining accurate client and provider records in database
Required Qualifications:
• Bachelor’s degree human services (preferred one year of experience to meet AIRS Certification); OR
• Associate’s degree and 2 years of experience working in the human service field OR
• High School diploma or GED with 4 years background working with aging and disability population, community health or hospital discharge planning
• Must become certified as an Alliance of Information and Referral Specialists (AIRS) within the AIRS standards.
Physical Demands:
• Essential functions of this position include: working with and around other staff and clients; vision acuity (near) for reading computer work; hearing and speech for ordinary conversation and for communicating with staff and clients in person and over the phone; finger dexterity / touch for typing filing etc.
• While performing the duties of this job, the employee is required to use computers and computer systems (including hardware and software) enter data, or process information for extended periods of time.
• Employee in this position will occasionally be required to access consumer’s homes that may require traveling distances and ascending /descending stairs, standing, bending and stooping.
• Occasionally may be required to attend training out of service area or out of state.
Work Environment:
• Noise level in the work environment is moderate.
• The position is located in a call center environment where at times may be able to hear conversations of other’s while attending to current call in process.
