The PUPPY described above is sold for the sum of $ _1948.50 __(Additional $250 for shipping)

BUYER is to initial on each line below, indicating that BUYER has read thoroughly and agrees to each statement.

______BREEDER does not guarantee this PUPPY to have a non-shedding coat or that it will be hypo-allergenic.

______All appropriate shots for the PUPPY’S age have been given, and a record of this vaccination history has been provided.

______This PUPPY has had a full physical exam and fecal exam performed by a licensed veterinarian, and was found to be in good health. A record of this health history has been provided.

______The BUYER agrees to bring this PUPPY to a licensed veterinarian within 72 hours of the PUPPYS arrival for a full physical exam and fecal exam at the BUYER’S expense. The BUYER agrees to provide the BREEDER all copies of this vet exam within 10 days of purchasing the PUPPY. If the PUPPY is found to be anything other than healthy by the BUYER’S veterinarian, it shall be returned to the BREEDER immediately, and the purchase price will be refunded. It is the BUYERS responsibility to return the PUPPY and any shipping fees acquired by the BUYER will not be refunded.

______If BUYER refuses to return the PUPPY, the BUYER assumes all responsibility of any vet bills acquired. The BREEDER will not at any time reimburse for vet bills acquired by the BUYER.

______Your PUPPY is health guaranteed against any serious life threatening genetic diseases for up to 2 years of the puppy’s age. If the PUPPY is found to have an inherited illness and it is confirmed by at least 2 veterinarians, the BREEDER will offer a refund of the purchase price or a replacement puppy from the next available litter (Breeders choice).

______Your PUPPY is guaranteed against Hip Dysplasia for up to 2 years of the PUPPYS age. The BUYER must provide the BREEDER with documentation from the Orthopedic Foundation for Animals (OFA) before a refund will be provided. The health guarantee for Hip Dysplasia will be terminated should the dog be allowed to become overweight at any point during the guaranteed period.

______The BREEDER reserves the right to have the PUPPY examined by a veterinarian of our choice prior to the replacement or refund.

______Refunds are ONLY provided when the puppy is returned for proven medical reasons and not for any other reason the BUYER may have for returning the PUPPY at any given time.

______All health guarantees apply only to the original BUYER of the PUPPY and transfer of dog ownership will void this contract.

______The BREEDER does not include in the health guarantee problems arising from trauma, accident, abuse, negligence, or failure to maintain general health care for the PUPPY.

______BUYER hereby agrees to maintain the PUPPYS health in good condition and to provide routine preventative health care including, but not limited to, examinations, vaccinations, internal and external parasites, medications, and any other usual procedures that will help assure good health. All veterinarian records must be provided if the BUYER is requesting a refund or replacement puppy. If any of the above preventative care was not provided or the vet records show the PUPPY was allowed to become overweight, all health guarantees in this contract will be void.

______The BUYER hereby agrees that if at any time the BUYER cannot take proper care of the puppy for any reason, the BUYER must contact the BREEDER and the BREEDER will have the first choice to either: (a) take the PUPPY back or (b) assist the BUYER in finding a suitable home for the puppy.

______This PUPPY is of pet quality only and the BUYER is prohibited to use this PUPPY for breeding purposes. The BUYER agrees this PUPPY will be spayed/ neutered by the PUPPYS age of 6 months old and veterinarian confirmation of this procedure must be supplied to the BREEDER by 7 months of age. Failure to do the above stated will void this contract and all health guarantees. Legal action will be pursued if the puppy is not spayed/ neutered by the PUPPYS age of 8 months old.

The BREEDER neither makes nor implies any warranties or guarantees, expressed or implied, other than those written in this agreement. This document constitutes the entire agreement between the BREEDER and BUYER with respect to this sale and any inconsistent oral terms have no effect on this contract. BUYER’S signature below indicates that he/she has read, agrees to, and does understand all the conditions of this Contract of Sale and Health Guarantee.
