Care Advisor

Position Purpose: / Care Advisorsassist olderpeople to remain living at home and are responsible for working in partnership with the consumer and/or their carer in receipt of Home Care Packages.
The objectives of the position is to promote consumer choice and control which includes:
- to assist people to remain living at home for as long as possible; and
- to enable consumers to have choice and flexibility in their care and support consumers in decision-making
- being responsive to their customs, beliefs and background
- promoting a wellness and enablement model
- supporting their independence and continuation ofparticipation within the community
Level 1 Care advisors do not provide clinical care or clinical assessments.
Level 2 Care advisors must maintain Division 1 RN registration with AHPRA, and are required to provide clinical care and assessment services as part of the Care Advisor role.
Qualifications & Requirements: / Mandatory:
Level 1
- Minimum 5 years’ experience in the community care sector as an assessor, or
- Relevant tertiary qualification, or
- Registered Nurse Division 2
- Experience in assessment and case management
- A current drivers’ license
- Registered Nurse Division 1 and current registration with AHPRA
- Experience in assessment and case management
- A current drivers’ license
- Community Services experience
- Case Management of complex care consumers
- Promotional and marketing experience
- Certificate in business related studies

The provision of care advisory services to Home Care Package consumers /
- Assessment and identification of consumergoals
- Development of consumer directed service plans
- Develop, review and implement service plans
- Timely commencement of services
- Monitor and review consumer goals and direction of their needs
- Support/mentor and educate consumers and activelyparticipate in the management of their Home Care Package as directed
- Skilling consumers and building capacity to self-manage their supports and services
- Monitoring of individual budgets and assist with consumer management of individual budgets in accordance with package level
- Assess consumers against the program’s eligibility criteria
- Design and implement appropriategoal direct care plans inresponse to identified needs
- Provide consumers with choices and flexibility in their care and services taking into consideration specific cultural needs
- Facilitate regular review meetings with consumers/carers and service providers to discuss the ongoing appropriateness of the plan of care
- Ensure that consumers exiting the program are provided with the supports required to ensure a smooth transition
- Accurately record data as required by MH HCC
- Liaise and document discussions with relevant service providers and agencies to improveservice provision for consumers
Clinical Services (Level 2 Care advisor only) /
- Conduct clinical assessments
- Deliver clinical nursing care
- Assist with the delivery of clinical education
- Assist with the supervision of Division 2 nurses and community care workers
- Assessments are conducted within timeframes prescribed in funding agreements and MH HCC program protocols
- Clinical care is provided, safely, effectively and in accordance with best practice principles and MH HCC policy, procedure and clinical guidelines
- Professional supervision and education is provided as required and in accordance with consumer needs and individual skills and experience
Quality and compliance /
- Ensuring Compliance with Legislative and Service Agreement
- Demonstrated commitment to Aged Care Reforms and Consumer Directed Care principles
- Adherence to the Home Care Common Standards
- Participation in the Australian Aged Care Quality Agency reviews
- Supports and services are arranged and delivered in accordance with Home Care Package Guidelines

Service Development /
- Attend local network meetings
- Attendance and membership at local service provider meeting
- To assist in the planning, implementation and evaluation of the Home Care Package Service
- To assist in the application process for Aged Care Approval Rounds
- Identify and develop strong relationships with key organisations requiring services.
- Expand new and current business by developing and maintaining relationships with the local networks, health professionals, and government
- Promote MHCC services through profile building opportunities including network meetings and community events
- Increase in referrals from case management agencies and other funded home care providers.
- High satisfaction feedback on consumer feedback surveys and reduced number of complaints from consumers/carers.
- Assist with the transition from Home Care Packages to Consumer Directed Care including the development and implementation of CDC Policies, Procedures and processes
Ongoing professional development /
- To participate in professional development opportunities according to identified personal/service requirements
- To actively participate and comply with annual Performance Reviews
- Attend external seminars and industry briefing sessions

Administration /
- To maintain comprehensive, up-to-date client records, utilising an electronic client record system
- To comply with MHCC policies and procedures
- Work in accordance with relevant policies and standards of MH HCC care advisory services
- Maintain accurate and up-to-date notes in the client management system
- Actively manage the individual consumer budgets including the monitoring of consumer expenditure ensuring service plans align with expenditure and within budget constraints
Takes reasonable care to protect the health and safety of themselves, fellow staff and others in the workplace. /
- Conducting pre-service home visits to assess consumers’ homes are safe for service provision.
- Complete incident reports.
- Elect and support health and safety representatives.
- Contribute to risk assessments.
