Medicinal cannabis guidance for physicians who practice in New Jersey

In order for physicians to register with the Medical Marijuana Program, they must:

1)hold an active New Jersey license that is in good standing

2)possess an active controlled dangerous substances registration that is not subject to limitation and

3) practice within the State of New Jersey.

To become a registered physician in NJ:

  • Go to
  • Select Physician Registration
  • Provide a personal and office contact information (although only name, office location, and phone number will be publicly displayed on the participating physicians list)
  • Provide your Controlled Dangerous Substance (CDS) and medical license information
  • Indicate that you have completed medical education in Addiction Medicine and Pain Management within the past 2 years. Type the course title or presentation attended
  • Enter desired user access information. This information will serve as your login to the registry system
  • Indicate that you adhere to the Physician Agreement and type your full name and title
  • Select Submit

The NJ Medical Marijuana Program will review your application and will respond via e-mail with further instructions for finalizing your application. Denied applicants will be provided instructions on amending applications.

Medical marijuana must be ordered by a registered physician from one of the Alternative Treatment Centers approved by the New Jersey Department of Health.New Jersey medical marijuana patients cannot grow their own marijuana plants.

Patients must have and maintain a bona fide physician-patient relationship with a physician who is registered with the Medical Marijuana Program. A bona fide physician-patient relationship is defined as a relationship in which the physician has ongoing responsibility for the assessment, care, and treatment of a patient’s debilitating medical condition whereby:

  • The physician-patient relationship has existed for at least one year; or
  • The physician has seen and/or assessed the patient for the debilitating medical condition on at least four visits; or
  • The physician assumes responsibility for providing management and care of the patient’s debilitating medical condition after conducting a comprehensive medical history and physical examination, including a personal review of the patient’s medical record maintained by other treating physicians reflecting the patient’s reaction and response to conventional medical therapies

Recommending physicians must certify that patients have an approved debilitating medical condition. List of approved debilitating medical conditions:

  • Amyotrophic lateral sclerosis
  • Multiple sclerosis
  • Terminal cancer
  • Muscular dystrophy
  • Inflammatory bowel disease, including Crohn’s disease
  • Terminal illness, if the physician has determined a prognosis of less than 12 months of life.
  • The following conditions apply, if resistant to, or if the patient is intolerant to, conventional therapy:
  • Seizure disorder, including epilepsy
  • Intractable skeletal muscular spasticity
  • Glaucoma
  • Post-Traumatic Stress Disorder (PTSD)

The following conditions apply, if severe or chronic pain, severe nausea or vomiting, cachexia or wasting syndrome results from the condition or treatment thereof:

  • Positive status for human immunodeficiency virus
  • Acquired immune deficiency syndrome
  • Cancer

A registered physician must complete and submit their patient’s Attending Physician Statement to the Medical Marijuana Program, which will return a Patient Reference Number and Patient Registry ID to the physician. The physician can then register their patient electronically at

Process for registering a patient:

•Login to using the username and password you created when registering for the NJMMP.

•Select the Physician Statement tab

•Select Patient Search

•Select Add Statement

•Provide patient information and debilitating medical condition

•Certify that patient has a qualifying medical condition for medicinal marijuana

•Select a 30, 60, or 90 day effective period. This time frame determines how many days a patient may purchase medical marijuana before being reevaluated.

Note: Physicians may authorize a maximum amount of two ounces of marijuana in a 30 day period.

•Verify the accuracy of all entered information

•Complete attending physician statement and submit to MMP registry

•Receive patient reference number from MMP

•Provide patient with reference number or a copy of their Attending Physician Statement to register with MMP.

•Upon expiration of the selected timeframe, you must re-assess the patient’s condition and determine whether to continue their use of medicinal marijuana for an additional 30, 60, or 90 day period.

•Upon the expiration, do not create a new physician statement for the patient. Edit the effective period in the original statement by: 1) Login, 2) Click Physician Statement, 3) Click Patient, 4) Select one of the three available options, 5) Click search, 6) Click on patient, 7) Select new effective date, 8) Choose the 30, 60, 90 day period, 9) Click agreement, 10) Click update.

Once the patient has their Patient Reference Number and a copy of their Attending Physician Statement, they will go to and follow the instructions to register online.
