E-Reader Borrowing Agreement

An e-reader, e.g. Kindle, Nook, etc., is a convenient, portable reading device. The size of a paperback it can hold a vast number of books. Users can highlight passages, take notes, and look up words in the dictionary. The York Middle School Library Program has acquired e-readers for student use. Student use is a PRIVILEGE. Students are expected to show extra caution and responsibility. E-readers, plus accessories cost approximately $200. For this reason, parental permission is required before an e-reader can be checked out to a student.

Student Agreement:

In addition to following the YSD Student and Internet Use** policy, I agree to take care of the YMS e-reader while it is checked out to me. I will use it for it’s intended use, i.e., reading e-books.

I will not:

•throw, drop, damage the e-reader in any way, stuff it into a full backpack or place it in the bottom of my backpack where it could become damaged.

•remove the e-reader from its protective case.

•give the e-reader to another student for his/her use.

•leave the e-reader unsupervised (on desk, in hall, in car, etc.)

•add or delete content to the e-reader.

•connect the e-reader to my personal computer at school or at home, or register the e-reader with a personal credit card to purchase items. If the library’s registration is disturbed by a user, the user will be responsible for the full replacement cost* of the e-reader.

I will:

•return the e-reader in good condition within the two week loan period.

•return all parts of the device, including the plug and protective case. Any missing items may result in charges.

•return the e-reader to a library staff person. If the e-reader is left in the book drop, or on the circulation desk, the user may lose future borrowing privileges of e-readers while a student at YMS.

Parent/Guardian Agreement:

I authorize my child, ______, to borrow an e-reader from the YMS Library.

•I understand the e-reader belongs to the YMS Library and my child will comply with the YSD Student Computer and Internet Use** policy, found on the school website, as well as the E-Reader Student Agreement (see below).

•I will help ensure the safe and timely return of the e-reader when it is due.

•I understand I am financially responsible for any loss of the device, willful or accidental damage to it, and for any charges resulting from unauthorized content downloaded to the e-reader.

•Further, I understand my child may lose future borrowing privileges of an e-reader if there is damage or the device is not returned in a timely manner.
