Objective

The objective of the reconnecting disadvantaged young people program isto:

•empower vulnerable young people from disadvantaged communities by providing them with support in the area of health and wellbeing, community participation, education, training and career pathways

•establish partnerships and the coordination of services to support and empower young people, including those from Pacific Islander backgrounds

•engage young people in the design, development and delivery of the empower youth programs in their local areas

•provide innovative, intensive and holistic service and program delivery to vulnerable young people in empower youth program, including the direct engagement of youth workers by funded organisations.

Description of the service

The reconnecting disadvantaged young people activity captures services provided under the Empower Youth program and the Young Pasifika program. The Empower Youth program provides three-year fixed term funding to local government and community sector organisations to support vulnerable young people in disadvantaged communities across Victoria.

As part of the program, young people will work with youth workers to complete an aspirational plan to guide their progress in four key areas of: health and wellbeing; community participation; education and training; and career development.

The Young Pasifika program provides two-year fixed term funding to the Centre for Multicultural Youth to support youth outreach workers to engage with Pasifika young people through existing community networks and relationships such as sport, churches and local government programs.

Funding also supports the building of community capacity and linkage to services which are vital to enriching the sustainability of community connectedness and sense of belonging for young people.

Client group

Disadvantaged or at risk young people aged 12-25 years old. This may include young people from refugee or multicultural backgrounds.

Young Pasifika program: Centre for Multicultural Youth

Service redevelopment

The department will continue to monitor service provision, and where necessary, may amend service specifications or funding allocations based on community needs or in response to changes to government policy and priorities.

Monitoring and review

Service Agreement

The Department of Health and Human Services monitors and reviews the organisation’s compliance with the Service Agreement’s requirements set out in the terms and conditions and schedules including the Applicable Departmental Policies. The Service Agreement is used to manage funding provided through the department for services delivered by community service organisations and local government authorities.

The Service Agreement includes two Applicable Departmental Policies:

  1. Department of Health and Human Services Policy and Funding Guidelines
  2. Service Agreement Information Kit<

The Service Agreement Information Kit provides information on the Service Agreement terms and conditions (chapter 3) and Applicable Departmental Policies (chapter 4). The Service Agreement Information Kit must be read in conjunction with the Department of Health and Human Services Policy and Funding Guidelines.

Key performance measure 1:The number of young people participating in the Empower Youth program per annum

Definition / The number of young people participating in the Empower Youth program, with a total of eight programs funded across Victoria in 2017-19.
Aim/objective / This measure provides information on the number of young people who participate in the Empower Youth program, and as such, provides an indicator of the participation rates of the target group.
